Remote RN Care Manager | Baraboo Area

Humana Inc. seeks an RN Care Manager to support Wisconsin Family Care Program members in Sauk County. You will assess health and safety needs, develop a Member Care Plan, and educate members on disease processes, nutrition, exercise and healthy living choices.

This role involves remote work with travel about 40% to visit members, coordinating with acute and primary care providers, and documenting in files within required timelines. Monday–Friday, 8:00 am–4:30 pm CST; driver requirements apply.
