Intensive Child Youth Case Coordinator

Support high-needs youth through strength-based planning and crisis intervention as a Full-Time Case Coordinator in Brantford, Ontario. Monday to Friday hours apply.

SOAR Community Services seeks an Intensive Child & Youth Case Coordinator for George House within its Housing Program. You will support youth aged 14-16 in care, using harm reduction and coordinated services. The work includes cognitive behavioural life-skills support and collaboration with partners.

Key Responsibilities:

• Manage client behaviours using Crisis Intervention Training techniques
• Facilitate strength-based service plans with clients
• Support Plans of Care with clinical lead consultation
• Advocate for clients and develop their advocacy skills
• Communicate concerns with parents, guardians, and referral sources

Requirements:

• College Diploma in Social Services preferred
• Current registration with an applicable governing body
• Minimum two years’ combined placement or paid experience
• Apply theoretical models to assess strengths, needs, and risk
• Valid G-level driver’s license and Canadian work eligibility
