Community Inclusion Support Worker

Support community participation through person-centered care, advocacy, and collaboration as a Community Inclusion Worker. Help a person pursue social, vocational, and volunteer goals.

Pacific Coast Community Resources seeks a Community Inclusion Worker to support independence and self-determination. You will coordinate community activities, uphold rights and choices, and maintain care, health, and safety standards. The work involves collaboration with families, professionals, coworkers, and community members.

Key Responsibilities:

• Respect and advocate for the supported person’s rights and choices
• Coordinate and facilitate social, vocational, and volunteer participation
• Empower the person to become a full community citizen
• Develop, implement, and document the Individual Service Plan
• Record daily activity and personal health information

Requirements:

• Grade 12 education and Community Support Worker Diploma
• Minimum two years’ community social services or healthcare experience
• Current Emergency First Aid, CPR, and Food Safe certificates
• Behavior management training and communication skills
• Unrestricted Class 5 license, reliable vehicle, and required clearances
