Position Summary
Registered Nurse with a passion for compassionate patient care is needed to join our Primary Care team. The Primary Care Nurse (PCN) practices within Northern Health’s vision of family‑centered care, applying professional standards and the BC College of Nurses and Midwives code of ethics. The PCN is a member of an interprofessional team, delivering safe and ethical care in physician practices, homes, schools and communities. Responsibilities include providing clinical follow‑up according to individual primary care plans, collaborating with the team to develop care plans and delivering direct clinical care to individuals, groups and communities.
Shift & Deployment
Day rotation, 08 : 00 to 17 : 48 (rotating). May be redeployed to satellite community clinics within the same community.

Primary Care Networks partner with family physicians, regional health authorities and local partners to redesign B.C.’s primary and community care system. Services in the Chetwynd Primary Care Clinic include :
- Public health nursing

Registration with BC College of Nurses and Midwives as a practicing RN or RPN.Three years recent, related clinical nursing experience in primary care or chronic disease management settings.Current valid B.C. Driver’s License.Skills and Abilities
