Job description

Relief Addiction and Mental Health Worker, HART Hub Direct Service - (OPSEU) Relief Role, Contract (End Date: March 31, 2028) London, Ontario (London Zone) - Application Deadline: September 29, 2026 - The London HART Hub is a low barrier recovery-oriented space that aims to support individuals experiencing homelessness with mental health and addiction needs to improve their health and wellbeing.

HART Hubs will support its clients through access to high quality evidence informed care that is respectful, trauma-informed, culturally appropriate and compassionate in nature.

It connects individuals with a continuum of care tailored to their needs by providing access to transitional supportive housing spaces where individuals can access a safe place to rest, obtain basic needs, health care, peer supports, employment and income supports, mental health and addiction services, and other services under one location.

Individual units allow opportunities for autonomy and independence while accompanying support services ensure basic and personal needs are met while the individual works towards treatment, recovery and wellness.

Relief Addiction and Mental Health Workers within the Hubs program, will assists clients with integrated care planning, system connection and navigation, onsite support with basic needs, mental health and addiction supports.

WHAT WE'RE OFFERING: What you'll be making - Starting rate for this role is $27.14 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ( $30.01 per hour for Level 1 – Direct Service, Regulated): Successful candidates who are registered with one of the designated professional colleges (e.g.

OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated) For internal applicants, grid level placement will occur as per the Collective Agreement Premiums of $2.00 will be applied for eligible Evening/Night and Weekend Shifts When you'll work - Relief shifts include days, afternoons, evenings, nights and weekends.

Your pension plan – Immediately eligible for optional enrollment in the Healthcare of Ontario Pension Plan (HOOPP) HOW YOU WILL BE MAKING A DIFFERENCE: Provide intake, screening, assessment and treatment planning Support clients on their journey towards physical/mental health improvement Ensure clients basic needs are being met (e.g., provision of food, showers, hygiene and a place to rest) Provide referrals to other supports and resources to maintain overall wellness Assist with the development of care plans, navigating local services, and engaging in proactive crisis intervention Provide one-on-one and group counselling sessions with a strengths-based wellness approach Ensure client record is updated with changes in circumstances, housing, medications, health conditions, diagnoses, etc Provide navigation assistance, connection to resources, and collaboration with established justice supports for those involved with the criminal justice system (pre-release support, probation or bail) Provide accessible options for communication for clients with varying needs and literacy levels (AODA, multiple languages, sign language, audio translation and referral to language classes) Assist with transportation needs upon intake, including obtaining travel assistance such as bus passes or other forms of travel, leveraging existing funding support Provide integrated care plans that encompass acute and primary care, mental health, addictions, income, transportation and safety Provide support for additional HUB sites as required Income and Employment Support: Provide guidance with income planning, applications (e.g., ODSP, OW, CPP), and tax filing Provide connections to employment support programs Provide strategies to access barriers, build confidence and improve employability Supportive Housing: Provide 24/7, on-site support to help individuals maintain stable housing at HART Hub Facilitate ongoing conversations around housing goals to secure suitable housing Provide rapid connection to housing stability resources WHAT YOU'LL NEED TO APPLY: Police Vulnerable Sector Check Requirements Successful candidates must have a Police Vulnerable Sector Check completed in the last six months.

Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 2-4 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.

We require the following qualifications: Completed post-secondary education in a related field of study.

  • ( Primary proof of credentials will be required during the hiring process, i.e.

an original degree/diploma/certificate or official transcript; for ongoing education, an unofficial transcript will be accepted) Valid G driver's licence and access to a vehicle with at least $2 million liability insurance to occasionally transport clients Ability to attend work regularly We prefer the following qualifications: Experience working with individuals with addictions or problematic substance use; mild, moderate and serious mental illness; no fixed addres Residential/community housing experience and familiarity with the Residential Tenancies Act.

Experience in crisis intervention/management and crisis resolution.

Experience working with individuals with an ABI or Developmental Disability.

Experience working in a community-based setting; Preferably with an established network of service & support resources.
