  • Conduct patient assessments and follow-up activities as required by the program.
  • Monitor patient progress and provide ongoing support throughout the treatment journey.
  • Maintain accurate and timely documentation within program systems.
  • Identify and escalate clinical or operational concerns as appropriate.
  • Contribute to quality improvement initiatives and program success measures.
  • Build and maintain positive working relationships with healthcare providers and clinic partners.

Basic Requirements

  • Diploma, degree, or equivalent combination of education and experience consistent with the P1 Professional Career Framework.
  • Strong verbal and written communication skills.
  • Proficiency using Microsoft Office and electronic documentation systems.
  • Ability to work approximately 2.5 days per week within program operating hours of 8:00 a.m. to 8:00 p.m. EST, Monday through Friday.
  • Ability to travel to clinics and healthcare sites throughout Alberta and British Columbia as required.
  • Excellent organizational skills and attention to detail.

Preferred Skills & Experience

  • Knowledge of reimbursement, patient access, or specialty pharmacy processes.
  • Experience providing patient education and clinical support.
  • Experience collaborating with physicians, specialty clinics, and multidisciplinary healthcare teams.
  • Familiarity with chronic disease management and treatment adherence programs.
  • Additional active nursing licenses in Canadian provinces.

Travel, Work Environment & Physical Requirements

  • Primarily remote/home-based role with travel as required.
  • Travel to clinics, healthcare facilities, and stakeholder meetings throughout Alberta and British Columbia.
  • Valid driver's license and access to reliable transportation may be required based on territory coverage.
  • Must be able to work independently in a remote environment while effectively managing competing priorities.
