POSITION SUMMARY: As an integral member of the hospital leadership team, the Clinical Manager, Rehab & Palliative Care (Patient Flow and Transitions) provides exemplary leadership and operational management for: Patient Access and Flow; Admitting; Alternative Level of Care; and the Transitional Care inpatient unit at Unity Health Toronto – Providence site. The Clinical Manager is responsible for the portfolio and its multigenerational/interprofessional team(s); ensuring excellence in the provision of patient care, the achievement of corporate/program objectives, the creation of a healthily workplace, program planning, quality management, participation in special projects, corporate/research initiatives and other activities that support and advance UHT goals. These accountabilities are facilitated through the provision of collaborative leadership, mentorship, education and communication within an inter-professional setting, to achieve measurable outcomes and performance targetswith an emphasis on patient, family and staff satisfaction and safety. MAJOR RESPONSIBILITIES: Participates and collaborates in planning, developing and delivery ofprograms and services within their portfolio; Develops, leads and implements short and long-term plans to support and advance portfolio priorities; Accountable for targets and outcomes of the areas of responsibility, as collaboratively identified by the leadership team; Provides leadership for their direct reports, including but not limited to: the interprofessional team, the administrative team, the flow and admissions team, the Clinical Operations Lead, and the ALC Operations Lead, in order to ensure the needs and preferences of individual patients are assessed and guide care planning; Works closely with theSenior Clinical Program Director, Providence Healthcare to ensure quality programming; Accountable for all Human Resource related activities including hiring, performance management, labor relations, and staff scheduling; Fiscally responsible for their budget as assigned, including variance reporting and management; Cultivates strong relationships and partnerships with internal and external interest holders, to meet the needs of the patient population, and to support patient flow pathways Responsible for the leadership and oversight of Alternative Level of Care (ALC) practices at Providence, including roles overseeing ALC, and participates in corporate ALC governance activities when required Maintains direct communication and collaboration with patients and families to ensure participation in and satisfaction with care inclusive of the process to address complaints and compliments when they arise; Liaises with external referral and care partners to ensure the admission, re-admission, consult and discharge process is well managed; Maintains a well-established relationship with the Inter-professional team and a good rapport with other departments within the organization; Responsible for their portfolio’s quality management system, inclusive of quality metrics: establishment, monitoring and associated improvement plan as needed, maintenance of the quality management board and portfolio huddles Collects, analyzes and disseminates quality data to the team to promote continuous quality improvement, maintaining accountability for the quality and integrity of data reported to external agencies Liaises and collaborating with physicians and physician leadership to support operations of clinical area(s) and organizational goals Responsible for establishing and maintaining communication channels to keep staff informed of the corporate quality improvement plan, patient safety plan, annual operating plan and strategic plan QUALIFICATIONS: Clinical background with registration in good standing with a recognized professional college required; Completion of a health profession related master’s degree, or appropriate recognized equivalent required; Minimum 5 years' leadership experience as an inter-professional team leader or manager in a similar health care setting required; Minimum 3 years’ leadership experience in managing patient flow, ALC practices/teams and transitional care required; Expert knowledge in discharge/transition planning, ALC legislation, and Ontario Health at Home practices required; Clinical knowledge/experience in a health care setting providing care/service for older adults an asset; Experience with patient flow, admissions practices and case collaboration required; Experience in managing within unionized environments; Demonstrated ability to work across teams, lead and influence outside your span of control, and collaborate in complex care environments Demonstrated ability to persuade and positively influence others; Excellent communication skills and computer skills; Proven ability to lead projects to full completion; Knowledge and understanding of LEAN methodology an asset; Very good attendance record; Very good performance record.
