Role Summary
This position covers three transitional care units in Oshawa and Pickering.
Responsibilities
- Execute the social and wellness care plan under the direct supervision of a Licensed Social Worker.
- Provide in‑person and virtual support to patients and families, including wellness checks, food and housing securities and social community support connections.
- Engage with community support services to follow up on referrals made by the social worker, ensuring referrals are actioned and a follow‑up date is established.
- Arrange and manage transportation services for patients to community appointments, and at times accompany them.
- Maintain and manage a caseload efficiently, ensuring priority code timeframes are met.
- Utilise a client‑centred approach to identify, prioritise, and monitor potential psychosocial problems and goals.
- Provide education and educational resources (online, in‑person, paper) that best meet patient needs to support specific conditions or social concerns.
- Advise clients and caregivers on available community resources and facilitate referrals when appropriate.
- Determine when further social work intervention is no longer necessary and assist clients in planning for discharge.
- Comply with continuous quality improvement initiatives.
- Adhere to Bayshore Policies and Procedures.
- Participate in quality activities and continuous improvement initiatives in keeping with the company’s Quality Management System.
- Participate in proactive health and safety activities.
- Maintain confidentiality of client and corporate information.
- Complete other tasks as requested.
- Participate in ongoing internal and external continuing education activities.
- Document and report all tasks and information according to Bayshore policy.
- Maintain treatment records in accordance with the Standards of Practice of the Ontario College of Certified Social Workers and Social Service Workers.
- Demonstrate agility in decision‑making and think critically to respond to a rapidly changing environment.
- Maintain positive and collaborative working relationships within the interdisciplinary health care team.
- Demonstrate the ability to be self‑directed and thrive in a dynamic environment.
- Remain calm under pressure and model composure.
Qualifications
- Possess a Diploma in Social Service Work.
- Be an active member in good standing of the Ontario College of Certified Social Workers and Social Service Workers.
- Provide police clearance that meets Bayshore Home Health policy.
- Possess general computer skills and have access to a computer with appropriate password protection and internet capabilities.
- Minimum of two years of community‑based therapy experience (preferred).
- Be able to drive, hold a current valid driver’s licence, and have access to a vehicle in good operating condition (preferred).
