OPSEU – Addiction Counsellor II, Emergency Department Addictions Patient Navigator

Job Title: Addiction Counsellor II - Emergency Department Addictions Patient Navigator (2 positions)

Status: Temporary Part Time

Department: Substance Use Disorders ICP

Program: Mental Health & Addictions

Location: Lakeridge Health Oshawa

Wage Schedule: $36.810 - $43.450 per hour

Lakeridge Health is a health network serving the Durham Region. This position supports the Substance Use Pathway Pilot and the Addiction Medicine Consultation Service at Lakeridge Health Oshawa, providing interdisciplinary care for patients in the Emergency Department presenting with substance use concerns.

The Addictions Navigator will offer support and addictions system navigation for ED adult and youth patients presenting with substance use and concurrent disorders. The Navigator will work with ED and MHA partners, Hospital to Home, Residential Withdrawal Management Services, and Rapid Access Addiction Medicine clinics to provide a continuum of evidence-based, patient-centred care. Functions include initial screening, consultation, intake services, assessment, referral, linking, advocacy, integration of required services, initial treatment planning and provision of services, and collaboration with community-based service providers.

Duties and Responsibilities

  • Maintain a professional collaborative relationship with programs within MHA, ED, Community Harm Reduction Programs, Hart Hubs, CMHA, DMHS and other community agencies.
  • Identify appropriate resources for patients and assist patients and families to navigate health care and social support systems.
  • Provide information and education to team members and the community regarding services across the continuum of care.
  • Review assessment information and prepare individualized treatment plans addressing strengths and goals.

Qualifications

  • Undergraduate degree in Social Sciences.
  • Demonstrated experience providing care to people who use substances.
  • Experience in screening, consultation, intake services; case management (assessment, referral/linking, advocacy, monitoring, treatment planning) and family supportive counselling.
  • Exceptional interpersonal, communication and organizational skills.
  • Knowledge of addiction theories, trauma-informed care, evidence-based treatment models and best practices.
  • Knowledge of issues related to treatment of substance use, harm reduction, concurrent disorders, crisis intervention, housing, family and women’s issues.
  • Ability to work with a clinical team and independently to high professional standards.
  • Knowledge of METAPHI recommendations, Health Quality Ontario SUD standards, and community resources.
  • Commitment to patient-centered, recovery-focused care.
  • Satisfactory performance and attendance record.
