The SPPCN is seeking a dedicated and compassionate Registered Nurse (0.6 FTE) for a unique and rewarding position in our Seniors Clinic . You will work with a multidisciplinary team to provide comprehensive, patient-centered care for older adults, supporting healthy aging, cognitive screening, chronic disease management, and proactive care planning. This is an excellent opportunity to practice full scope, apply clinical expertise, and make a meaningful difference in the lives of seniors within the community.
This position offers a choice of schedule :
- Monday, Thursday, Friday
- Tuesday (12 : 00–8 : 15 pm), Thursday, Friday
Responsibilities
Assessment of patient conditions, needs and goals in collaboration with the patient, attending physician and interdisciplinary teamIdentify needs and set priorities in collaboration with the client and other team membersProvide short term interventions as neededFacilitate appropriate referrals to community resources and programsProvide relevant patient education to prevent and manage clinical conditionsParticipate in evaluation of the clinic and its servicesAssist in planning, developing, and monitoring for services providedAssist in the development of policies and protocols for chronic disease management, geriatric care, maternal / child and mental healthDevelop systems and communications protocols to facilitate patient follow upAssist in setting up health promotion programs relevant to the needs of the practice populationAssist in developing and implementing patient self-management programsAssist in implementing strategies for improving the quality and provision of care for the practice populationAny other duties reasonably required for the success of the PCNEducation & Experience
Baccalaureate degree in Nursing preferredFive years or more of related nursing experienceCurrent CRNA Registration, CPR, and BCLSInitiative, accountability, decision-making, interpersonal and communication skillsAbility to work in a dynamic environment with flexibility and balancing prioritiesExperience in PAP testingExperience working in a team with other health professionalsAdvanced skills in caring for patients with chronic, complex, and / or specialized needsExperience establishing protocols for program startups and workload managementKnowledge of nursing processes and practices including intervention strategies and therapyStrong written and oral communication, high level of perceptivenessCritical thinking skillsAbility to build working relationships with healthcare professionals, clients and community membersAbility to prioritize and manage time in a busy environmentRespect and understanding of confidentialityKnowledge of medications and ability to conduct medication reviewsCustomer service orientation; thrives in a changing environmentIntegrity and professional behaviourBasic computer skills and knowledge of electronic medical recordsPersonal Attributes and Competencies
