DESCRIPTION DU POSTE

Le service des consultations externes recherche une infirmière autorisée, qui sera placée sous la responsabilité du ou de la responsable clinique du Programme ontarien de psychothérapie structurée. La personne titulaire de ce poste sera responsable d’assurer les services d’accueil, ainsi que d’évaluer et de trier toutes les demandes d’aiguillage reçues dans le cadre du Programme.

La personne retenue fera partie d’une équipe interdisciplinaire chargée de la coordination des aiguillages, notamment : le triage de toutes les demandes d’aiguillage reçues dans le cadre du programme; la communication avec les sources d’aiguillage afin de s’assurer que les renseignements reçus sont complets et pertinents; la communication avec les personnes aiguillées afin de compléter les renseignements requis; et l’accompagnement et l’orientation de ces dernières. L’aide au triage pourra être fournie aussi bien en personne que par téléphone. La personne titulaire de ce poste fournira également des renseignements et aidera les personnes, les familles et les autres professionnels de santé à s’orienter dans le système.

Conditions Requises

  • La maîtrise du français est obligatoire.
  • Détenir un certificat de compétence valide de l’Ordre des infirmières et infirmiers de l’Ontario (B. Sc. inf. de préférence).
  • Être membre en règle de l’Association des infirmières et infirmiers autorisés de l’Ontario est un atout.
  • Une certification de l’Association des infirmières et infirmiers du Canada (AIIC) en soins infirmiers psychiatriques et en santé mentale constitue un atout.
  • Démontrer une connaissance des normes de pratique de l’Ordre des infirmières et infirmiers de l’Ontario et un engagement à respecter les normes de pratique réflexive telles que décrites par l’Ordre sont requis.
  • Démontrer une connaissance avérée des normes canadiennes en matière de soins infirmiers en psychiatrie et en santé mentale.
  • Détenir un certificat de réanimation cardio-pulmonaire pour les professionnels de santé valide.
  • Posséder d’excellentes aptitudes en service à la clientèle.
  • Posséder des connaissances en thérapie cognitivo-comportementale.
  • Être à l’aise d’utiliser des outils de mesure cliniques.
  • Posséder au moins 5 ans d’expérience en psychiatrie au sein d’un établissement ou d’un service de santé mentale.
  • Posséder au moins 3 ans d’expérience dans le domaine des soins infirmiers d’urgence.
  • Posséder une bonne connaissance des ressources communautaires en santé mentale et en toxicomanie.
  • Être en mesure d’orienter efficacement les usagers, ce qui témoigne d’une connaissance approfondie des ressources communautaires disponibles dans la zone d’intervention du Programme ontarien de psychothérapie structurée et de solides compétences en matière de collaboration avec les partenaires communautaires et les professionnels de santé.
  • Posséder une excellente connaissance des examens de l’état mental, ainsi que des évaluations psychosociales et des situations de crise, ainsi que la capacité de les réaliser.
  • Démontrer une connaissance des normes de pratique de l’Ordre et un engagement à respecter les normes de pratique réflexive telles que décrites par l’Ordre.
  • Démontrer une connaissance du domaine de la santé mentale, ainsi que des politiques et de la législation en la matière, telles que la Loi sur la santé mentale et la Loi sur la protection des renseignements personnels sur la santé.
  • Être capable de travailler efficacement de façon autonome et au sein d’une équipe interprofessionnelle.
  • Démontrer une connaissance approfondie des médicaments psychotropes.
  • Faire preuve de discernement et d’un comportement mûrs et efficaces dans des situations d’urgence.

Job Description

Outpatient Services is seeking a Registered Nurse, reporting to the Clinical Manager of the Ontario Structured Psychotherapy Program (OSP). This position will provide central intake services and will assess and triage all referrals received for the OSP.

The successful candidate will be part of an interdisciplinary team responsible for the coordination of referrals including: triaging of all referrals received to the program; communication with referral sources to ensure complete and appropriate information is received; communicate with the individuals referred to complete information required and provide support and navigation to those referred. Triage support will may be provided both in person and via the telephone. This position will also provide information and system navigation to individuals, families and other care providers.

Requirements

  • Bilingual in French is required
  • A current certificate of competence from the College of Nurses of Ontario, (BSc N preferred)
  • Registered with the RNAO in good standing is preferred
  • CNA certification in psychiatric/mental health nursing is an asset
  • Demonstrated knowledge of College of Nurses of Ontario (CNO) Standards of Practice and commitment to maintaining reflective practice standards as described by the CNO is required
  • Demonstrated knowledge of Canadian Standards for Psychiatric-Mental Health Nursing
  • Current CPR-HCP is required.
  • Excellent customer service skills required.
  • Cognitive Behavioral Therapy knowledge required.
  • Comfort in utilizing clinical measurement tools.
  • 5 years psychiatric experience in a mental health facility or service.
  • 3 years’ experience in working in crisis nurse role.
  • Sound knowledge of community resources in mental health and addiction.
  • Ability to make effective referrals, reflecting in depth knowledge of community resources within the OSP Catchment area and strong collaborative skills in working with community partners and health care providers.
  • Thorough knowledge and ability to administer mental status examinations, psychosocial and crisis assessments.
  • Demonstrated knowledge of the College Standards of Practice and commitment to maintaining reflective practice standards as described by the College.
  • Demonstrated knowledge of Mental Health, and relevant policies and legislation, such as the Mental Health Act and PHIPA is required.
  • Ability to work effectively individually as well as part of an interprofessional team.
  • Demonstrated advanced knowledge of psychotropic medication.
  • Proven ability to exercise mature, effective judgment and behaviour in emergency situations.
