Job description
Step into the role of Primary Care Physician at Reena Health Services, providing holistic care within a Community Health Centre. Focus on patient support, health equity, and interdisciplinary collaboration. This position requires you to offer comprehensive medical care to clients of all ages, integrating assessments, treatments, and ongoing support. Your collaborative work with an interprofessional team will help manage health conditions and provide critical community services. You will also participate in after-hours on-call coverage as part of the team model. Key Responsibilities: • Manage acute and chronic health conditions effectively • Develop care plans in collaboration with clients and team members • Maintain comprehensive medical records and documentation • Conduct regular case reviews and interdisciplinary consultations • Engage in quality improvement initiatives and community health programming Requirements: • Medical degree and good standing with CPSO • 3-5 years of community-based primary care experience • Certification in Family Medicine is an asset • Strong leadership and communication skills • Commitment to professional growth and team collaboration Support community health initiatives and provide impactful care as a Primary Care Physician with Reena Health Services. #J-18808-Ljbffr
