Diabetes Footcare Nurse
We provide primary health care through a range of options including prevention assessment diagnosis and treatment. Our interprofessional teams include health professionals educators and support workers representing both traditional Indigenous and contemporary models of care and using wholistic health and community development concepts.
The Diabetes Footcare Nurse is part of a client-centered multi-professional team structure and reports to the designated Interprofessional Team Manager. The position assists with the development and implementation of a culturally appropriate inter-professional diabetes strategy that promotes self-care and is responsive to diverse needs. This position is part of the Ontario Nurses Association Bargaining Unit.
NOTE: Traditional practices involve cultural ceremonies and burning sacred medicines including tobacco sweetgrass sage and cedar within the work setting.
PRIMARY DUTIES / RESPONSIBILITIES:
QUALIFICATIONS:
Certificate of successful completion of an Advanced Nursing Foot Care course that is in alignment with the Canadian Association of Foot Care Nurses National Competencies for Advanced Nursing Foot Care in Canada or willingness to obtain through employment
Current registration as a Registered Practical Nurse with the College of Nurses of Ontario
Demonstrated cultural competencies to fully understand how to direct and facilitate the development and implementation of a culturally sensitive diabetes strategic care plan
Experience and demonstrated ability to work within a wholistic care model; able to work independently and be an effective team member
Experience working in an Indigenous organization and ability to understand and speak Anishinaabemowin a definite asset
Willingness and ability to travel
Valid drivers license and own vehicle required; copy of current drivers abstract required
Current criminal/vulnerable persons records check
Commitment to ongoing training and professional development relevant to job requirements
Proof of immunization in compliance with policy requirements is mandatory
Positive attitude and capacity to act as a healthy lifestyle role model
Salary: $30.85-$38.91
per hour with excellent benefits including HOOPP pension plan. Salary is in accordance with the Ontario Nurses Association Contract (ONA).
