A growing, community-based organization that provides shelter and outreach support for homeless youth in Peel Region. They are a leading organization recognized in the community for its commitment to providing youth in need with a safe environment and access to services to help them achieve independence and foster growth.

The Pod Group is partnering with this organization to place a resourceful, compassionate, and hands-on individual to fulfill the role of a ACT Team Supervisor.

WHY YOUR ROLE MATTERS 🔍

The ACT Team Supervisor provides oversight and supervision for youth case management staff within an Assertive Community Treatment (ACT) model. This role will ensure high-quality, evidence-informed service delivery aligned with Housing First for Youth principles and the strategic priorities of Our Place Peel.

The Supervisor supports the ACT team members in delivering coordinated, youth-centered care to young people ages 16-24 who are experiencing complex mental health, substance use, and housing challenges, with a focus on addressing barriers to housing. The role combines staff supervision, clinical consultation, and a small, focused caseload of high-acuity clients to maintain direct practice and model best practices.

HOW YOU WILL MAKE AN IMPACT 🎯

Leadership & Supervision

  • Provide day-to-day supervision, coaching, and performance management to case management staff.
  • Facilitates regular staff meetings to review program deliverables for quality and continuous improvement.
  • Participate and support the recruitment process for front-line roles such as Intensive Case Managers and Diversion workers.
  • Support staff in delivering high-quality, youth-centered case management services.
  • Help coordinate team case consultations and maintain daily oversight of assigned care load for the team.
  • Ensure care loads are equitably distributed on the team.
  • Promote staff development through training, mentorship, and ongoing feedback.
  • Ensure adherence to organizational policies and strategic priorities, ACT model fidelity, and HF4Y principles.

Supervisory Oversight & Practice Excellence

  • In coordination with the Youth Clinical Services Manager, provide consultation on complex cases (mental health, substance use, housing instability).
  • Review and support development of assessments and individualized care plans.
  • Ensure service delivery reflects Housing First for Youth trauma-informed, harm reduction, and recovery-oriented practices.
  • Support staff in crisis response, risk assessment, and de-escalation.
  • Monitor service quality through team case reviews, documentation audits, and outcome tracking.

ACT Team & System Coordination

  • Collaborate with the larger Youth Services Department, including Prevention and specialized services, to ensure integrated care planning and delivery.
  • Support care path planning and service coordination across health, housing, justice, and community sectors.
  • Participate in and help guide case conferences to ensure alignment and minimize service gaps.
  • Serve as a key liaison with internal and external partners, advocating for youth assigned to the care team.

Program Operations & Quality Assurance

  • Oversee team caseloads and support effective workload management and prioritization.
  • Ensure timely, accurate documentation and reporting in line with requirements.
  • Contribute to program evaluation, quality improvement, and reporting.
  • Support implementation of best practices and continuous improvement initiatives.
  • Promote a healthy and safe environment, while maintaining process, procedure and policies.
  • Provides on-call support, which includes problem solving, consultation, guidance and feedback to front-line employees through crisis management and challenging events.

Direct Service

Manage and support high-acuity or complex youth to:

  • Maintain clinical practice expertise and credibility.
  • Model best practices and engagement strategies for staff.
  • Provide direct intervention in particularly complex or high-risk cases.
  • Deliver assessment, care planning, and case management support consistent with HF4Y and ACT principles.
  • Support staff and youth during crisis situations, housing instability, and transitions.

WHAT YOU WILL BRING 🎓

We value diverse professional and lived experiences, including backgrounds in mental health, addiction, and homelessness.

  • Bachelor’s degree in a relevant field. Master’s and/or Certification in Mental Health and Addictions is strongly preferred.
  • Registration with a professional regulatory body (e.g., RSW, RCC, Canadian Certified Counsellor, OACYC, or equivalent).
  • Minimum 3–5 years of progressive experience in mental health, substance use, and/or homelessness services.
  • Minimum 1–2 years of supervisory or leadership experience.
  • Knowledge of housing first for youth, trauma-informed care, and harm reduction practices.
  • A leader with excellent communication, presentation, organization and interpersonal skills with a passion for supporting the most vulnerable and marginalized youth in our community.
  • Knowledge of resources and supports for youth and exposure to Region of Peel resources.
  • Experience working with youth and/or HF4Y or ACT models strongly preferred.
  • Experience with program development, evaluation, and quality improvement.
  • Flexibility to work in a 24-hour, 7 days a week operation, while supporting an on-call rotation.
  • Valid CPI/UMAB and First Aid/CPR certifications.
  • Valid Ontario Driver’s License, clean driver’s abstract, access to a personal vehicle and $2,000,000 (minimum) liability car insurance coverage.
  • Satisfactory Vulnerable Sector Police Check that meets the standards of the organization.
  • Must be flexible to work at Brampton and/or Mississauga location.
