Reporting to the Manager Pharmacy Clinical Programs or designate the Clinical Pharmacist Community Health Services works collaboratively with the Seniors End of Life and Home & Community Care teams is responsible for practicing client centered care by identifying and resolving potential drug therapy problems in the community setting; provides medication counseling and drug information to clients and health care providers; conducts clinic sessions and/or home visits as required; and provides academic detailing to optimize prescribing practices.
Licensed by or eligible for Licensing by the College of Pharmacists of British Columbia.
Completion of an Accredited Pharmacy Residency and three (3) years recent pharmacy practice experience including community based practice or an equivalent combination of education training and experience.
