Clinical Pharmacist Services

  • Individualized pharmacotherapy
  • Counselling patients about their medication therapy
  • Work in collaboration with physicians, nurses, and other healthcare providers to develop, implement, and monitor therapeutic plans
  • Assist in the development of drug therapy guidelines for individual disease conditions
  • Prepare and submit case studies and articles for publication in professional journals
  • Act as department resource person within your assigned area
  • Participate in medication use evaluations, quality improvement initiatives, and clinical drug trials
  • Provide mentorship and supervision to pharmacy students, residents, and new staff
  • Ensure prescriptions are validated and dispensed accurately; resolve medication safety concerns and document professional practice activities proficiently

  • Current registration as a Pharmacist with the College of Pharmacists of British Columbia
  • Completion of an accredited Hospital Pharmacy Residency Program (or equivalent combination of education, training, and experience)

Detailed Overview

The Clinical Pharmacist provides clinical pharmacy services, including individualization of patient medication therapy; patient medication counselling; consultation with physicians, nurses, and other healthcare providers; and assistance in the development of drug therapy guidelines for individual disease conditions. Provides support to the Pharmacy & Therapeutics Committee, contributes to professional journals, and acts as a department resource person. Performs drug distribution duties, validates prescriptions, identifies safety concerns, and documents all care activities.

Responsibilities

  • Consult with physicians, nurses, and other healthcare providers to develop and discuss therapeutic plans appropriate for each patient; ensure documentation is maintained in the patient health record.
  • Individualize patient medication therapy through literature review, chart and laboratory review, and patient assessment.
  • Provide patient medication counselling, explain procedures, and assess understanding.
  • Assist in the development of drug therapy guidelines and monitor their effectiveness.
  • Participate in ward rounds, program team meetings, and unit‑level pharmacy services.
  • Act as department resource person, providing detailed drug information as needed.
  • Contribute case studies and review articles to professional journals.
  • Provide instruction and/or supervision to pharmacy students and residents.
  • Participate in quality improvement activities, audits, and process recommendations.
  • Perform drug distribution duties, including interpretation of medication orders and dispensing using a computerized system.
  • Review prescriptions for therapeutic suitability and clarity.
  • Communicate with physicians, patients, and team members for prescription clarification and problem resolution.
  • Ensure medications are dispensed accurately and records are completed.
  • Participate in medication use evaluation projects and maintain inventories.
  • Participate in clinical drug trials and review adverse reaction cases.
  • Participate in medication safety initiatives and quality control activities.
  • Assist in staff meetings, committees, and other programs; identify opportunities for improvement.
  • Maintain records for narcotic and controlled drugs and departmental statistics.
  • Prepare reports and maintain computerized files and spreadsheets.
  • Perform other related duties as assigned.

Qualifications

Education and Experience

Current registration as a Pharmacist with the College of Pharmacists of British Columbia. Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experience.

  • Ability to apply clinical pharmacy knowledge and make decisions with minimal supervision
  • Ability to communicate effectively, both verbally and in writing
  • Ability to establish and maintain effective working relationships with other health practitioners, patients, family members, and the public
  • Ability to organize and prioritize work
  • Ability to operate related equipment
  • Physical ability to carry out the duties of the position
