Job Summary:
The Registered Practical Nurse/Diabetes Educator functions as a team member to deliver diabetes education to clients, healthcare teams and their support lay persons on an individual and group basis. This can involve, but is not limited to, client assessment, intervention and evaluation, program development, liaison/coordination of existing community services, education and prevention promotion to the community and healthcare providers, related quality assurance and activity, as well as maintaining a strong knowledge base of all aspects of diabetes management.
He/she demonstrates critical thinking skills and sound decision making based on scientific principles in theory. Is responsible and accountable for all activities in which he/she engages. The Registered Practical Nurse will be assigned duties and responsibilities in line with the scope of practice determined by the College of Nurses of Ontario. The RPN is responsible to quickly identify when the patient’s condition has gone beyond the scope of the RPNs practice or has unexpectedly changed to the RN for assessment.
Qualifications:
Education:
Certifications and Registrations
· Ontario College Diploma in Practical Nursing and registration in good standing with the College of Nurses of Ontario is required.
· Certified Diabetes Educator (CDE) or in process of obtaining a CDE certification
Skill, Ability & Experience:
· Three (3) to five (5) years of recent acute care experience will be preferred.
· Demonstrated ability to work collaboratively in a multidisciplinary team environment including internal and external partners is required.
· Demonstrated strong leadership ability is required.
· Demonstrated facilitation, critical thinking and problem-solving skills is required.
· Demonstrated exceptional organizational skills are required.
· Demonstrated ability to be self-directed and thrive in a dynamic environment is required.
· Active and proven engagement in continuous quality improvement is required.
· Experience with Meditech Expanse is required.
· Knowledge of Microsoft Office is required.
· Demonstrated knowledge of the various bed types within Blanche River Health, individual admission criteria unique to each unit, i.e., Acute Surgery/Medicine, Complex Continuing Care, Rehabilitation, Assess-Restore Beds, Mental Health etc. is preferred.
· Bilingualism in French and English is preferred.
