Job descriptionAs an Occupational Therapist on the Supportive Living Team, you will provide Home Care services to clients living in Continuing Care Type B and Continuing CareTypeB Secure Space Homes within the Southwest part of Central Zone. You will be part of a multidisciplinary Allied Health team and have the opportunity to collaborate with and develop effective partnerships with other providers. Responsibilities
Provide Home Care services to clients in Continuing Care Type B and Secure Space Homes. Collaborate with the Allied Health Area Manager, Team Lead, and Occupational Therapy Qualified Practice Lead to support service delivery. Lead clinical direction for a Therapy Assistant and mentor Occupational Therapy or Therapy Assistant students when appropriate. Assess individual needs, provide therapeutic interventions, follow‑up, and program evaluation to ensure optimal client outcomes. Deliver evidence‑informed services focusing on occupational performance for individuals, groups, or communities. Act as a direct service provider, consultant, educator, or advocate within the health care team. Engage individuals, groups, and communities to enable meaningful and chosen occupations, including self‑care, productivity, and leisure. Provide comprehensive services including triage, screening, consultation, assessment, diagnosis, cooperative goal setting, treatment, prevention, self‑management, education, case management, referral, transition support, and collaboration with stakeholders. Include clients and families as integral members of the health care team and encourage their active participation in all aspects of care. Participate in interprofessional teams, quality improvement initiatives, peer mentoring, and supervision of students or assistants. Qualifications
Completion of a bachelor’s degree in Occupational Therapy from an accredited university program. Registered with the Alberta College of Occupational Therapists (ACOT). Heart&Stroke: BCLS‑HCP certification required. Current AADL Authorizer preferred.
