The pharmacist coordinates clinic medication management systems; serves as a dedicated clinical resource in addictions medicine, infectious diseases, and chronic disease management; supports transition of care, including hospital discharge follow-up; and liaises with community pharmacies and other healthcare partners to improve medication access, continuity, adherence, and therapeutic outcomes.

The role supports patients through medication assessment, prescribing and deprescribing activities, chronic disease management, addiction medicine, transitions of care, medication access, patient education, and interdisciplinary consultation.

  • Collaborate with physicians, nurses, community pharmacies, specialists, and external partners to optimize medication therapy, address medication-related barriers, and support continuity of care.
  • Provide medication-related consultation, education, and clinical support to interdisciplinary team members and contribute to patient and provider learning opportunities.
  • Support clinic medication management systems, medication safety practices, policies, workflows, and regulatory compliance requirements.
