The Program Coordinator - Community Oncology Education and Care Transitions, works under the direction of the Manager and is responsible for leading the planning, development, implementation, and evaluation of educational programs and initiatives for primary care providers, cancer surgeons, CCMB staff, and Community Cancer Program staff across Manitoba. The role advances health equity, cultural safety, and trauma- and violence-informed practice as foundational principles, with a specific focus on improving cancer outcomes for First Nations, Métis, and Inuit peoples and other equity-denied populations.

This position leads the coordination for the Transitions of Care program, overseeing quality control of existing clinical processes and supporting the implementation of new transitions throughevidence-based approaches and consensus building with clinical teams. The role also leads the coordination of a provincial patient education materials program, managing the full lifecycle of materials development from intake through review, approval, and publication.

Working collaboratively with the program team, education coordinators, and a broad range of provincial interest holders, the coordinator ensures that educational programs, transitions of care initiatives, and patient resources are responsive to identified needs, grounded in CCMB’s clinical priorities, and aligned with the CCMB Roadmap and Accreditation Canada requirements. The incumbent coordinates grant submissions, reporting, and workplans, and provides direct day-to-day supervision to the Community Oncology Program Education team.

Qualifications:

Education

  • Post secondary degree in Health Sciences, Social Services or Nursing is required from an accredited educational institution.
  • Post-secondary education in project management is strongly preferred.
  • Masters degree preferred

Experience

  • 3 years project management experience - implementing in a healthcare or other relevant environment •
  • 2 years of supervisory experience
  • 2 years program planning and evaluation experience
  • 2 years experience in planning educational programs or events for adults •
  • 1 year of experience with writing applications for grant funding and progress reports, developing and managing work plans and budgets

An acceptable equivalent combination of education and experience may be considered

Skills and Abilities

  • Experience in change management in a health care setting is strongly preferred
  • Demonstrated experience in developing, leading, implementing and evaluating new projects/initiatives in a healthcare or clinical setting
  • Understanding of the importance of clinical outcomes and pathways
  • Knowledge of and experience with qualitative and quantitative data analysis, evaluation and research methods
  • Strong problem solving, critical thinking, and organizational and time management skills
  • Strong written and oral communication skills, including public speaking and presenting, as well as strong attention to detail
  • Establish and maintain positive and effective working relationships with all levels of staff, the general public, as well as internal and external interest holders and partners
  • Demonstrated ability to collaborate, liaise with, and support physicians, nurses, nurse navigators and other clinical staff / health professionals •
  • Work in independent and team environments
  • Demonstrated understanding of health equity, social determinants of health, and structural barriers—including systemic racism and the impacts of colonization—that may affect access to care and treatment, with specific understanding of equity-deserving populations
  • Demonstrated facilitation and presentation skills
  • Skilled in working with the internet, Windows platforms and Microsoft programs including Outlook, Word, Excel, PowerPoint, Teams, and Project
  • Completion of, or demonstrated commitment to completing, Indigenous cultural safety and anti-racism training
  • Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per Truth and Reconciliation Calls to Action (2015) and Manitoba's Path to Reconciliation Act (2016)

Preferred:

  • Certificate or degree in Adult Education
  • Knowledge of transition of care points in the clinical care pathway
  • Knowledge of the cancer care system and experience in the continuing education of health professionals
  • Experience working in a primary care or cancer surgery setting
  • Experience with electronic health records (EMR)
  • PMP, PRINCE2 or PMBOK certification an asset
  • PROSCI certification an asset
  • Understanding of and experience working to address the impacts of colonization, systemic racism, and historical trauma on the health and wellbeing of impacted individuals and communities
  • Experience working with First Nations, Métis, and Inuit communities and/or equity-deserving populations
  • Knowledge of Indigenous cultural safety and anti-racism actions, inquests and reports, e.g., Jordan's Principle, Joyce's Principle, and the Brian Sinclair Inquest

Required Licenses, Registrations, Designations

  • Active member in good standing of an associated regulatory body as appropriate

Physical Requirements

  • May be required to travel throughout the service area requiring a valid Manitoba driver’s license and use of a personal motor vehicle.
  • May be required to work occasional evenings and weekends o Some travel may be required.

  • Ability to speak French is considered an asset
