Support and Resource Coordinator

Full-time (35 hours per week)

The Alzheimer Society of Peterborough Kawartha Lakes Northumberland and Haliburton is seeking a caring and compassionate individual to join our team as an Outreach Support and Resource Coordinator.

The Outreach Support and Resource Coordinator plays a key role in supporting individuals connected to the organization by providing ongoing support program coordination and connection to resources.

In this role youll be a welcoming point of connection for care partners and individuals living with dementia to help ensure that no family feels alone on their dementia journey.

Key Responsibilities

As an Outreach Support and Resource Coordinator you will:

  • Build and maintain supportive relationships with care partners and individuals living with dementia to identify evolving needs and connect families to appropriate supports.
  • Provide information guidance and practical support to help people navigate available programs and community services.
  • Coordinate specialized initiatives including Project Lifesaver and Finding Your Way Ontario.
  • Coordinate volunteers involved in counselling programs.
  • Provide back-up coverage for care partner support groups and social programs.
  • Provide administrative support for counselling program operations and special projects.

Education & Experience

  • Diploma or degree in Social Services Social Work Psychology Gerontology Human Services Nursing or related field.
  • 2-3 years experience working in healthcare social services community services.
  • Experience working with older adults people living with dementia and their care partners
  • Experience in assessment care planning and service coordination



Skills & Competencies

  • Strong knowledge of dementia care and person-centred supportive counselling practices
  • Excellent interpersonal skills
  • Strong organizational and time-management skills
  • Cultural sensitivity and diversity awareness
  • Knowledge of community health and social service systems in the region
  • Strong written communication skills
  • Proficiency in Microsoft Office client databases and social media platforms



This Role Requires

  • Travel throughout the four-county region
  • Valid G Class drivers license and access to a reliable insured vehicle
  • Vulnerable Sector Police Check (within the last 2 months)
  • Membership in a relevant professional regulatory body is considered an asset
  • Position requires flexibility to work evenings and weekends based on program needs



Working Conditions

  • This role includes a combination of work in office clinic community and home settings.
  • Travel across the four counties is required.
  • The nature of this role requires flexibility in scheduling including the ability to begin work earlier or end later than regular office hours (8:30 a.m. 4:30 p.m.) to support travel time to programs. These scheduling adjustments are required periodically based on program and community needs.

Our Compensation Package

  • Grade 4 position with starting salary commensurate to experience
  • $22.00 - 27.00 per hour (35 hours per week)
  • 15 days vacation in addition to office closure between December 24 and January 2. All vacation hours are accessible to you within three months of your employment with us.
  • 18 sick days per year 5 of which may be used for family obligation reasons. All sick time hours are accessible to you within three months of your employment with us.
  • 5 paid and 5 additional unpaid emergency leave days per year
  • Flex schedule options; During summer months flex time may be leveraged to take Friday afternoons off to enjoy the weather and all that living that our 4 counties offer
