Job description
Join Ontario Health atHome as a Float Care Coordinator, delivering comprehensive patient assessments and service linkages. This temporary full-time position supports holistic patient care across multiple sites. In this critical role, you will engage with patients, caregivers, and healthcare practitioners to determine care needs and eligibility for services. Your mission will be to establish helping relationships while ensuring that patient values are honored. This position requires a strong organizational aptitude to manage diverse patient care efficiently. Key Responsibilities: • Collaborate with patients to assess care needs • Establish and monitor personalized care plans • Connect patients with suitable service providers • Maintain clear communication throughout care delivery • Balance patient choices with services and resources Requirements: • Active membership with a regulatory body as RN, RSW, etc. • Minimum of 2 years in community health • Excellent organizational skills for a busy environment • Valid driver's license and reliable vehicle access • Experience with documentation and report writing Make an impact on community health with your professional skills and compassion. #J-18808-Ljbffr
