JOB SUMMARY:

The Clinical Education and Community Program Manager (CECPM) is a member of the hospice leadership team responsible for the strategic and operational leadership of clinical education and community outreach programs in support of high-quality hospice and palliative care delivery.

The CECPM leads the development, implementation, and evaluation of evidence-based education, quality improvement initiatives, and community outreach services that strengthen the capacity of internal teams and community practitioners. This role oversees specialized clinical education and outreach functions, supports student placements, fosters collaborative partnerships across the healthcare continuum, and ensures programs are aligned with organizational priorities, regulatory standards, and evolving community needs.

KEY ACCOUNTABILITIES & SUCCESS CRITERIA Clinical Education

Leads the organization’s clinical education strategy in collaboration with clinical leaders and department managers.

Oversees the Clinical Coach and Palliative Pain and Symptom Management Consultant (PPSMC) roles to identify practice gaps and support development of educational initiatives to strengthen clinical knowledge, competency, and professional practice.

Leads the planning, development, delivery, and evaluation of education programs for hospice staff and community partners with the support of the Director and Residence Clinical Manager.

Coordinates student placements within the department in collaboration with academic institutions, internal teams, and clinical leaders.

In collaboration with the Clinical Manager, supports orientation and planning for ongoing education of new and existing staff.

Encourages research and evidence-informed practice initiatives that advance nursing and interprofessional education, contribute to knowledge translation, and enhance clinical practice and program outcomes.

Outreach Services and Community Programs

Provides leadership for outreach initiatives, including palliative care consultation services and grief and bereavement programs.

Leads program planning, implementation, evaluation, and continuous quality improvement activities to ensure responsive and sustainable community services.

Builds and maintains collaborative partnerships with healthcare providers, community agencies, and external stakeholders.

Ensures outreach services align with organizational priorities, regulatory requirements, and community needs.

Leadership and Program Management

Upholds the Code of Ethics and the Regulated Health Professions Act of Ontario.

Participates in hospice leadership meetings, annual operational planning meetings, and committees as required.

Supports the Director, Stedman Community Hospice with departmental planning, reporting, and operational priorities.

Participates in cross-coverage of the hospice leadership team and Manager-On-Call rotation.

Organizes clinical staff meetings as appropriate.

Completes performance enablement activities for staff in accordance with organizational policies.

Develops strategies to support recruitment, retention, engagement, and professional growth of clinical staff while fostering a positive team culture, collaborative working relationships, and a respectful, collegial work environment.

Assists with the development and management of clinical education and outreach budgets to ensure effective resource utilization.

Participates in the development, review, and revision of departmental policies, procedures, and clinical resources.

Ensures policies, practices, and resources effectively support staff and program needs.

Collaborates in the development of the annual Quality Improvement Plan through the identification of key performance indicators (KPIs) and engagement with staff, patients, and families, and leads the ongoing development, monitoring, and reporting of KPIs aligned with departmental operational plans and organizational strategic priorities to evaluate program effectiveness, support data-informed decision-making, and drive continuous quality improvement.

Completes necessary preparatory work for departmental accreditation reviews to ensure ongoing readiness and alignment with accreditation standards.

With the support of the Director, leads initiatives that advance diversity, equity, inclusion, and Indigenous reconciliation by identifying opportunities to improve service accessibility, strengthen culturally safe care practices, and enhance organizational capacity and service delivery across hospice and community programs.

Works collaboratively with the leadership team to review the emergency preparedness plan and support related education for staff as required.

Works in collaboration with the Community and Volunteer Engagement Coordinator to support the development and integration of specialized volunteer roles that enhance education, supportive care, and outreach programming within community services.
