Position Summary In collaboration with the Central South Stroke Network, the District Stroke Coordinator will provide strategic and operational leadership to support an integrated stroke system in Brant Haldimand Norfolk. The incumbent will report to the Director of the Stroke Program and will ensure the provision of best‑practice stroke care across the continuum of the healthcare system (promotion, prevention, hyperacute, acute, rehabilitation, and community programs).
Primary Responsibilities
Develop and sustain formal partnership throughout the district related to stroke care.
Oversee and manage the development of the stroke strategy in consultation with appropriate internal and external stakeholders.
Assume responsibility and accountability for overall project administration, including professional, technical, staff, and financial aspects.
Maintain, monitor, and interpret program reports and statistical data.
Develop work plans and progress reports.
Disseminate information to all participants.
Facilitate implementation of continuing education activities as required.
Develop and maintain an annual budget.
Develop business cases and impact analysis for new projects, programs, and resource allocation.
Mandatory Qualifications
Bachelor of Health Science, Nursing, or Allied Health is required.
Regulated Health Professional and Member in good standing with their respective College is required.
Minimum of 2 years of stroke experience required.
Progressive leadership experience in the healthcare system preferred.
Experience in health service research, program evaluation, or project management.
Demonstrated ability to work both independently and as a member of a team.
Critical thinking, communication, organization, problem‑solving, teaching, and leadership skills.
Knowledge of data collection and information systems with strong computer and IT skills.
Understanding of existing processes and mechanisms related to the continuum of stroke care as described by the coordinated stroke strategy.
Supportive of principles of patient‑focused care.
