The Nurse Practitioner works both independently and collaboratively as part of the interprofessional team, to provide a full spectrum of healthcare services including assessment, diagnosis, management and health promotion to patients, clients and families. The Nurse Practitioner will provide quality care through advanced practice, education, research, leadership and consultation as outlined by hospital policies and procedures and the College of Nurses of Ontario Professional and Practice Standards to support the provision of transitional and seniors care.
Skills And Qualifications Required
Current unrestricted registration as a Registered Nurse, Extended Class (Primary Health Care or Adult)
Master's degree in Nursing
Completion of an approved and accredited Nurse Practitioner Program
Two (2) years of recent related experience in a healthcare setting
Current CPR certificate
Experience in the care of older adults
Experience in mental health care specialty
Experience in student supervision, an asset
Experience in research, quality improvement or project implementation, as asset
Position Responsibilities
Works to full scope of practice as an advanced practitioner within the auspices of approved medical directives, policies and procedures of Providence Care
Completes admission, discharge, prompt assessments/stabilization, diagnostic and therapeutic interventions, and timely feedback to the interprofessional team as well as patients and clients and family members
Holds the primary responsibility and accountability as Most Responsible Practitioner (MRP) where assigned
Consults and collaborates with physicians and the interprofessional care team to optimize patient care.
Initiates and performs a wide variety of investigations, diagnostic procedures and contributes to development of collaborative care plans; Interprets diagnostic and screening tests
Completes, documents and communicates comprehensive and focused physical and mental health assessments on clients
Acts as a clinical resource for members of the interprofessional team keeping abreast of current legislation, regulations, best practices, professional and practice standards
Facilitates client/family access to resources to meet their goals and provides health and wellness teaching and chronic disease management as necessary
Participates in providing education, mentoring and supervision to student Nurse Practitioners
Participates in continuous quality improvement and accreditation activities
Leads, collaborates or participates in nursing or other approved research projects
Collaborates with Professional Practice to advance nursing practice through provision of in-services and continuing education
