Canadian Mental Health Association Durham is an integrated community mental health and primary care hub providing services to Durham Region for over 60 years. The organization delivers a broad range of services—including information, education, case management, psychosocial rehabilitation, housing, and primary healthcare—through inter‑professional teams that address social determinants of health using a recovery model.
Overview of the role
Provides outstanding primary care nursing services for marginalized clients who may have severe and persistent mental illness, across the lifespan. The position is community‑based.
Responsibilities
- Participate in the full scope of nursing services as a member of a clinical team for all clients.
- Function within the Mission United hub model in all daily encounters.
- Complete triage assessments.
- Perform comprehensive mental health assessments.
- Complete holistic assessments that include relevant medical, psychiatric and social domains.
- Prepare individualized nursing care plans that reflect the client’s goals.
- Provide information and resources on prevention, healthy lifestyle, sexually transmitted diseases, diabetes, nutrition, alternative lifestyles, chronic diseases and terminal illnesses as appropriate.
- Apply a harm reduction approach on a regular basis.
- Participate in treatment monitoring and evaluation of care.
- Plan for end‑of‑life care where indicated.
- Educate clients on their medication, including side effects and interactions to foster adherence.
- Perform venepuncture and wound care on a daily basis.
- Facilitate access to addictions services, including individual and group counselling, withdrawal management and follow‑up.
- Exchange and/or transfer new knowledge about evidence‑based practices within the team.
- Administer medication/treatments (including IM route) to clients and maintain appropriate documentation.
- Manage multiple nursing interventions simultaneously in rapidly changing situations.
- Direct plans of care for highly complex clients in collaboration with the multidisciplinary team at Mission United.
- Assist in the development and revision of policies related to nursing health care.
- Provide linkages to other health care professionals and community services as needed.
- Assess changes in the client’s clinical status and intervene when necessary to minimize risk to client and staff.
- Document client contact using clinical methods within predetermined time frames in accordance with CMHA Durham policies and procedures.
- Respond to emergencies as a member of the Emergency Response Team.
- Perform other tasks as assigned by management.
- Intermediate proficiency in oral and written French is an asset.
Risk Management
- Minimize risks by adhering to CMHA Durham policies and procedures, identifying, assessing, evaluating, and controlling risks to ensure the well‑being of staff and clients.
- Complete medication reconciliations ongoing with clients.
Other
Comply with Mission United’s vision, fundamentals, policies, procedures and values. Apply all CNO standards into practice on a daily basis, including documentation, standardized information management practices, professionalism, ethics, etc.
Skills you bring
- Registered Nursing Diploma or Degree and a minimum of two years’ related mental health experience or equivalent combination of education and experience.
- Membership in the Ontario College of Nurses’ Association and the Registered Nurses’ Association of Ontario.
- Sound knowledge of the Mental Health Act, Health Care Consent Act, internal agency and other related legislation in Ontario.
- Sound knowledge of psychotropic medications including usage and potential side effects.
- Ability to develop and maintain positive relationships with clients, community, partners, staff, and to represent the agency in a professional manner.
- Ability to acquire and utilize community resources effectively to meet client needs.
- Excellent written and verbal communication skills and ability to formally present information to individuals or groups for nursing care plans and meetings.
- Ability to resolve complex client problems and ensure a high standard of nursing service.
- Excellent assessment and organizational skills.
- Flexibility to adapt to change.
- Ability to work independently when necessary.
- Ability to think critically and creatively.
- Ability to make accurate nursing assessments and implement best practices.
- Valid Ontario Driver’s license, access to a dependable vehicle in safe working order and appropriate insurance coverage.
- Knowledge of MS Office (Word, Excel, PowerPoint, Access and Outlook) and internet.
- Up‑to‑date CPR and First Aid certification.
- Ability to work a variable schedule which may include evenings and weekends as required.

- Requires concentration, attentiveness, and critical thinking to assess clients, administer medications/injections, attend meetings, develop care plans, prepare documents, and use nursing equipment. May lift walkers or push wheelchairs as needed.
