Job Description
- Provide ministry‑funded, community‑based physiotherapy services within clients’ homes and/or through virtual service delivery
- Complete comprehensive physiotherapy assessments using interviews, standardized tools, and informal assessment techniques in areas including:
- Geriatrics
- Neurology
- Orthopedics
- Palliative care
- Develop and implement goal‑oriented, evidence‑based treatment plans in collaboration with clients and caregivers
- Assign, supervise, and monitor physiotherapy assistants, when appropriate
- Provide education to clients and caregivers and connect them to appropriate community resources
- Maintain accurate, timely, and professional documentation, including:
- Assessment findings
- Care plans
- Progress and discharge summaries
- Documentation aligned with referral source requirements and College of Physiotherapists of Ontario (CPO) standards
Qualifications
- Registered Physiotherapist in good standing with the College of Physiotherapists of Ontario (required)
- Valid G2 or G driver’s license with access to a reliable vehicle for community travel
- Community‑based physiotherapy experience is an asset, however new graduates are welcome
- Ability to work independently while collaborating within an inter‑professional team
- Strong organizational skills, including self‑scheduling, prioritization, and meeting reporting deadlines
- Excellent verbal, written, and report‑writing communication skills
- Proficiency with computer applications, including MS Office Suite and proprietary clinical software
