This opportunity is with our Good Shepherd – Seniors' Support Services, Seniors at Risk (SAM) Program.
How You Will Make an Impact
Reporting to the Program Manager, the Program Planner enhances the dignity and quality of life of clients, caregivers, visitors and staff by supporting seniors and adults with special care needs to remain living as independently and safely as possible within the community.
The Program Planner will also be responsible for:
- Implementing standardized leisure and recreation assessments, including chart reviews and interdisciplinary collaboration.
- Developing, coordinating and supervising recreational and therapeutic activity programming for clients.
- Leading group and individual activities while encouraging participation and monitoring changes in client functioning and abilities.
- Supporting virtual, telephone-based and technology-assisted recreational programming as required.
- Preparing and distributing activity and care packages to clients and caregivers when applicable.
- Maintaining program spaces and equipment in accordance with infection prevention and control (IPAC) standards.
- Managing the annual activity program budget and purchasing approved supplies and equipment.
- Assisting clients with activities of daily living including mobility, eating, toileting, dressing and transfers as required.
- Participating in program evaluation, quality improvement initiatives and interdisciplinary team planning.
- Collaborating with internal programs and external community partners to support integrated client care.
- Supervising students, volunteers and delegated program staff.
- Maintaining accurate documentation, attendance records, statistics and program reporting.
- Providing service in a dignified, culturally safe and respectful manner that reflects anti-racism, anti-oppression and harm reduction principles.
- Observing all health and safety rules in compliance with the Occupational Health and Safety Act and organizational policies.
What You'll Bring to Our Team
- Diploma and/or Degree in Recreation, Leisure Studies or a related field.
- Demonstrated experience working with seniors and/or individuals with dementia and complex care needs.
- Knowledge of recreation programming, leisure assessments and therapeutic activity planning.
- Ability to work independently while contributing effectively within an interdisciplinary team environment.
- Strong leadership, organizational and problem-solving skills.
- Excellent verbal and written communication skills.
- Experience supervising students, volunteers and/or program staff.
- Knowledge of Continuous Quality Improvement and Risk Management principles.
- Computer literacy including experience with email, internet and word processing applications.
- Current First Aid, CPR and WHMIS certification required.
- Food Handler's Certificate required.
- Pursuing or maintaining registration with Therapeutic Recreation Ontario (TRO) and/or related professional associations is considered an asset.
- Demonstrated commitment to anti-racism, anti-oppression, cultural competence and harm reduction frameworks.
