Job description
Role: Primary Care Nurse Practitioner – Community Health
Company: TAPP Workforce Solutions
Facility: Community Health Organization
Location: Scarborough, ON
Position Type: Part-Time | 1-Year Contract | 7 hours/week
Salary: $52.83 – $74.83/hour | May include evenings, weekends, and on-call

High-Level Overview for Candidates

TAPP Workforce Solutions is recruiting a Primary Care Nurse Practitioner on behalf of a community health organization serving diverse populations in Scarborough. This role focuses on delivering comprehensive, family-centered primary care to unattached patients, with an emphasis on health promotion, disease prevention, and equitable access to care.

Key Responsibilities
  • Provide comprehensive primary care across the lifespan within NP scope
  • Assess, diagnose, and manage acute and chronic conditions
  • Deliver preventive care, including immunizations
  • Develop and implement patient-centered care plans
  • Collaborate within an interdisciplinary team
  • Support marginalized populations with complex health needs
  • Participate in health promotion and outreach initiatives
  • Maintain clinical documentation and ensure confidentiality
  • Contribute to quality improvement initiatives
  • Participate in on-call rotation and occasional home visits
Requirements

Education
  • Master’s degree in Nursing
Certifications
  • Primary Health Care Nurse Practitioner (PHCNP) certification
  • Registered in good standing with the College of Nurses of Ontario (Extended Class)
Experience
  • Experience working with diverse and underserved populations
  • Experience in community health or primary care settings (preferred)
