POSITION SUMMARY
:Collaborating with professionals in the Chronic Disease Education Program this position will conduct literature searches, organize findings, support planning and development of clinical decision tools related to chronic disease management team practice and decision tool use. The work undertaken as above will facilitate creation of a needed pillar for safe and successful team based primary care management of chronic conditions such as diabetes, COPD, and heart failure.
