As a Program Lead, Therapy, we are looking for a Therapy professional who is an innovative thinker, has interest in modernizing home care through integrated care delivery, and has experience in program management.
You will play a key role in providing consultation, assessment, and interventions for diverse client groups across Ontario. The Program Lead (PL) will be involved in the development of programs, mentorship of new staff and ongoing coaching for Therapy teams, professional development and quality and risk management. The Program Lead will promote a culture of continuous improvement and will
facilitate the generation of innovative ideas and patient care design for CarePartners.

What The Role Involves
Developing and supporting specialized Therapy programming and Ontario Health atHome programsOrienting new staff to discipline specific policies, procedures, resources and toolsSupporting staff Therapists’ annual performance reviews,, onboarding processes and annual check-ins as required.Supporting with caseload reviews and quality of care reviews as neededChairing / co-chairing and organizing regular discipline-specific clinical meetings throughout theyear
Being a resource for urgent clinical issues that may arise for therapists and assistantsIdentifying clinical trends, departmental tools / resources and learning needs to meet clinicalrequirements
Developing discipline-specific forms and templates to support care deliverIdentifying education needs of the Therapy programs based on quality of care reviews, risk events, performance reviews and programs requirementsCo-interviewing candidates for hire with the Clinical ManagerSupporting operational leaders with recruitment, retention, mentorship, performance reviewsCreating and maintaining onboarding and orientation program development / delivery incollaboration with learning and development teams and operational leadership
Designing and collaborating with regional leaders on future models of care and growthopportunities and projects
Evaluating and monitoring Therapy program metrics and outcomes, identify trends andcollaborate on improvement opportunities and risk mitigation strategies
Other duties as requiredWhat You Bring
Regulated Allied Health professional, such as Occupational Therapist and Physiotherapist given preference.Master’s degree preferred.
Current registration with the applicable regulatory college in the province of Ontario2 years of supervisory experience and / or experience within home health service deliveryExperience with quality and risk management, and program delivery including data analyticsCritical thinker with excellent written and verbal communication skillsPractical experience with mentorship and coaching. An understanding of adult learningprinciples is an asset
Ability to effectively establish and build rapport with internal and external parties, includingcommunity partners
Proficiency with Microsoft Office 365, Teams, and digital health platformsCarePartners In Your Community
In addition to providing home-based personal support, rehabilitation, therapy, nursing and palliative care across Ontario, CarePartners also serves the community through clinics, transitional care units, and provides relief in nursing homes and shared care settings. Through our Community Nursing Services outreach program, we’ve been organizing medical care and clinics in countries with poor access to health care since 2009.
