Program Summary: The Path Home – Pine Villa is a short- term transitional housing program for hospital patients who no longer require acute care but still require care and support in preparation for a long-term living arrangement.

Pine Villa specializes in supporting older adults (55+) with complex physical health challenges, mental health challenges, addiction or cognitive impairment.

The Path Home – PIne Villa, provides client centered and goal-oriented support services that enable clients to continue their recovery in the community as they wait for permanent housing. The Path Home – Pine Villa is a unique partnership between LOFT Community Services, SPRINT Senior Care and Sunnybrook Life Sciences Centre.

It is funded by the Ontario Ministry of Health and Long Term Care.

This partnership reflects the transformation of health care in Ontario which brings the expertise and some of the resources of a hospital setting to a community based transitional housing program.

Some of these resources include on site clinical practitioners including an Occupational Therapist, Physiotherapist, Nurse Practitioner and Advanced Practice Nurse.

Job Summary: The Registered Nurse (RN) will be part of an interdisciplinary team, who is expected to ensure the safety and security of the resident environment, to practice in accordance with College of Nurses of Ontario professional standards, and the policies and procedures as defined by LOFT Community Services.

The RN will hold a unique and highly independent role as the clinical lead in an innovative practice setting.

The RN will lead a team of Registered Practical Nurses (RPNs) to ensure that applicable delegated or controlled acts are safely and consistently delivered by Personal Support Workers (PSWs).

The RN will be responsible for working with individuals from diverse backgrounds, lead clinical supervision of clients in the program; and providing training and leadership around the physical, cognitive, mental health and social/recreational needs of older adults and seniors in a way that enhances their dignity and personal empowerment.

The RN is directly responsible for clinical assessment and monitoring of client needs, assigning and monitoring controlled act delegation activities between RPNs and PSWs, and providing training and supervision to PSWs in the performance of delegated and controlled acts. What You Will Do: Conduct comprehensive clinical assessments and develop individualized nursing care plans to support high-quality, client-centered care.

Provide clinical oversight, training, and monitoring of delegated nursing acts across multiple program locations, ensuring regulatory compliance, accurate documentation, and timely reporting.

Lead and support nursing practice by monitoring RPN performance, providing clinical guidance, and delivering direct nursing care when required.

Oversee medication management, including administration, physician order transcription, medication reconciliation, MAR maintenance, and adherence to pharmacy policies and professional standards.

Assign, coordinate, and monitor delegated care activities between RPNs and PSWs Deliver training, mentorship, and supervision to PSWs in the safe and effective execution of delegated and controlled acts.

Act as a clinical resource to staff by providing expert advice, coaching, and support on healthcare-related concerns and best practices.

Foster therapeutic relationships with clients using Recovery-Oriented and Psychosocial Rehabilitation approaches.

Collaborate with interdisciplinary teams, community partners, and service providers to ensure seamless, coordinated care and positive client outcomes.

Maintain confidentiality and ensure compliance with organizational policies, infection prevention protocols, and professional practice standards.

Participate actively in team meetings, case conferences, and mandatory training sessions Provide backup support to Registered Practical Nurses, as required.

Contribute to a collaborative, professional, and client-focused team environment.

Perform other duties and special projects as assigned by leadership.

What You Bring: Good standing with the College of Nurses of Ontario (CNO); RNAO membership preferred.

University undergraduate degree in Nursing.

Minimum 3 years of experience in home and community-based nursing.

Proven experience supporting older adults and seniors with complex mental health, cognitive, addiction, and responsive behavioural challenges.

Strong knowledge of recovery-oriented care, psychosocial rehabilitation principles, and therapeutic relationship building.

Experience delegating and overseeing controlled nursing acts (e.g., wound care, diabetes management, G-tube care, mechanical lifts) performed by unregulated healthcare providers.

Demonstrated ability to provide clinical leadership, support knowledge transfer, and mentor interdisciplinary care teams.

Strong understanding of evidence-based nursing practices, clinical assessment, and complex case management.

Comprehensive knowledge of community healthcare services, the Ontario healthcare system, and available social supports for adults and seniors.

First Aid and CPR certification required.

Strong clinical judgment, problem-solving, and decision-making skills in complex care environments.

Excellent communication, coaching, and relationship-building abilities with clients, families, staff, and community partners.

Ability to work independently while collaborating effectively within an interdisciplinary team Skilled in practice education, staff development, and quality care oversight.

Proficient in documentation, reporting, and the use of healthcare technology and computer systems.

Commitment to continuous learning, professional development, and best-practice care Strong understanding of cultural competency, diversity, equity, and anti-oppression principles.

Multilingual abilities and experience working with diverse, multi-barriered populations are considered strong assets.

Work Environment: Physically able to perform demanding body mechanics (i.e.

lifting, transferring, Hoyer, etc.) Occasional evening and weekend hours may be required.

Physically able to perform occasional client transfers in the home (as part of the assessment process).

Extensive travel is involved and use of own vehicle is necessary.
