Job description

The Niagara Falls Community Health Centre (NFCHC) is a non‑profit, community‑governed organization dedicated to providing accessible, comprehensive health services at no cost to residents of Niagara Falls. NFCHC offers a wide range of programs including primary care, dental services, mental health counseling, nutrition guidance, occupational therapy, and specialized support for 2SLGBTQ+ individuals, refugees, and newcomers. The Centre emphasizes health promotion and community development through free workshops and outreach initiatives designed to address social determinants of health. With a multidisciplinary team of healthcare professionals, NFCHC is committed to empowering individuals and fostering a healthier, more connected community.

Position Summary

Reporting to the Clinical Director, the Registered Nurse will provide primary health care services to medically and socially complex clients within the Registered Nurse Scope of Practice. The Registered Nurse will lead quality care, promote access to care, and in collaboration with the Clinical Team identify efficiencies in care. The incumbent will provide a range of primary care services which includes treatment, assessment, and health education. The incumbent may also provide point of care in other community settings and through home visits; the incumbent will work cooperatively with all CHC staff and external service providers.

Accountabilities and Responsibilities

  • Duties and responsibilities include, but are not limited to:
  • Practice to full scope of the RN professional standards and deliver primary care including, but not limited to: allergy injections, immunizations, well‑women assessments, physical assessments and screening, triage, history taking including new patient intake information, chronic disease health education/counselling, assessment and treatment of STIs, assessment and treatment of UTIs, wart removal, tuberculosis skin testing, and point of care testing.
  • Provide initial patient assessment and responsible to inform and consult with members of the Clinical Team.
  • Responsible to provide and lead health education and teaching while remaining informed of clinical guidelines and recommendations.
  • Triage patients for acuity/urgent appointment needs.
  • Acts as a mentor for new RNs, RPNs including providing orientation, assisting with schedule development in partnership with the Clinical Director to ensure new RNs/RPNs are receiving appropriate onboarding/training.
  • Supervise student learners by providing orientation, learning opportunities, and reviewing/completing learning plans with students.
  • Maintain clinical supplies and equipment with timely inventory checks and orders, to ensure clinic is equipped, inclusive of medical equipment, medication samples, etc.
  • Maintain vaccine fridge including temperature logs and reporting to Public Health and maintaining inventory.
  • Perform phlebotomy on‑site.
  • Provide point of care in other community settings as identified by Leadership.
  • Maintain collaborations with medical supply companies and pharmaceutical representatives.
  • Responsible to maintain high quality standards and timely documentation in the Electronic Medical Records (EMR)
  • Participate in NFCHC Committees as needed and appropriate for the role and the needs of NFCHC
  • Act as member of an interdisciplinary team which assists with the development, analysis and implementation of client programming and services to deliver primary care services.
  • Provide input for development and implementation of NFCHC’s programming and services.
  • Participate in quality improvement activities as identified by the NFCHC.
  • Develop and maintain strong ties to other professionals and community members.
  • Work in a manner that demonstrates commitment to NFCHC’s mission, vision, and values.
  • Contributes to the CHC’s strategic goals and objectives and values the equality and diversity of the client community and NFCHC staff.
  • Participate in professional development opportunities as identified in the performance management process.
  • Adhere to the expectations of NFCHC policies and procedures and operating procedures.
  • Work in a safe manner as per relevant health and safety legislation and NFCHC policies.
  • Ability to work extended hours, weekends, and other hours as requested by leadership.
  • Any other reasonable duties as required.

Education and/or Work Experience Requirements

  • Current Registration, and be in good standing with the College of Nurses of Ontario
  • Post‑secondary academic credentials in Nursing from a recognized institution
  • RN Prescribing Certification would be an asset
  • Certified Diabetes Educator would be an asset
  • Experience in working in a community health setting would be an asset
  • Certifications relevant to target clientele (addictions, newcomer health, gender affirming care, acute and complex care) also an asset
  • Experience working with people who are marginalized, socially isolated, living in poverty, and with complex physical and mental health needs, including newcomers and refugees, senior citizens, youth, individuals and families with low incomes, and people experiencing homelessness.
  • Experience working with a diversity of clients and their families or supports in managing chronic diseases such as diabetes, asthma, cardiovascular disease, etc.
  • Proficiency in the use of EMR systems to follow established protocols and maintain accurate and timely documentation.
  • Knowledgeable with resources within the community and methods of access.
  • Maintains an understanding of the health issues of the community to provide appropriate services.
  • Proven ability to provide compassionate, respectful, non‑judgmental and culturally competent care to the disenfranchised and primary populations.
  • Demonstrated communication, time management, clinical, and organizational skills.
  • Previous experience establishing collaborative working relationships with a multi disciplinary team, clients and community service providers.
  • Ability to problem‑solve and apply sound judgment.
