  • Monitor and document client progress, observations, and responses to treatment (with OT/PT review and oversight)
  • Assist with transfer training, safe lifting techniques, and mobility aid usage
  • Provide caregiver education and support for OT/PT‑directed care strategies
  • Assist with basic equipment set‑up, environmental safety recommendations, and home visit follow‑up tasks
  • Maintain appropriate communication with supervising OT/PT, office staff, and interdisciplinary team members
  • Travel to client homes across the GTA and maintain flexibility as caseload changes

  • Negative TB test completed within the last year
  • Valid CPR/First Aid certificate
  • Valid WHMIS2015 certificate of completion
  • COVID‑vaccination receipts subject to an exemption pursuant to Ontario's Human Rights Code
  • Immunization for measles, rubella, varicella, tetanus, and diphtheria required; hepatitis B immunization strongly urged
  • Valid criminal record check completed within the last year
  • Ability to identify and report client care concerns to Care Coordinators when needed
  • Requires good judgement, ability to work independently, and ability to work cooperatively with an interdisciplinary team
  • Current/valid Health and Supportive Care Providers Oversight Authority (HSCPOA) registration is an asset

  • Previous experience in home and community care is strongly preferred
  • Strong communication skills and comfort working independently in varied home environments
  • Ability to follow OT/PT treatment plans accurately and safely
  • Reliable transportation and willingness to travel across the GTA
  • Physically capable of supporting clients during mobility, transfer, and exercise‑based interventions
  • CPR/First Aid, vulnerable sector check, and immunization compliance may be required

Benefits

  • Compensation: $28‑35 per hour
  • Flexible scheduling that can be adapted to your availability
  • Supportive rehabilitation team with strong PT leadership
  • Meaningful work in a community setting where you directly impact client mobility, safety, and independence
