St. Joseph’s Lifecare Centre is the largest Long-Term Care Facility in Brantford and Brant County with 205 beds and is focused on providing high-quality care to our residents.
We are seeking a compassionate and enthusiastic Nurse Practitioner to join our organization that shares in our Mission, Vision, and Values.
The Nurse Practitioner will be an integral part of a collaborative and inter-disciplinary team focused on delivering exemplary care to our residents.
The primary role of the Nurse Practitioner is to direct, provide and evaluate holistic patient care for a specified patient population.
The Nurse Practitioner will provide leadership to promote program development process improvements as well as the enhancement of professional standards within the Clinical Team at SJLCB.
The Nurse Practitioner will act as a clinical resource for nurses and other Regulated Health Care Professionals.
Key Accountabilities & Success Criteria
- Completes comprehensive health history and physical assessment
- Identifies and orders diagnostic tests and procedure according to approved medical directives within the NP scope of practice
- Collects, interprets and analyzes data from multiple sources and uses this data to formulate medical diagnoses within scope of practice
- Develops implements and documents the plan of care for the patient
- Works collaboratively with patients, physicians and other members of the interdisciplinary team to develop a plan of care
- Prescribes pharmacological and non-pharmacological therapies according to approved medical directives.
- Evaluates and revises plan of care based on patient response
- Performs skills and invasive / non-invasive procedures related within scope
- Initiates interventions for the purpose of stabilizing patients in urgent and emergent situations
- Develops cohesive working relationships with interdisciplinary providers in our community, both internally and externally
- Leads in the development and revision of scientifically based clinical standards for a selected patient population
- Works as a team member to evaluate compliance with professional standards
- Participates with nursing administration in the hiring, supervising and evaluation of nursing staff
- Participates and provides leadership on committees and task forces at all levels of the organization
- Participates and provides leadership in the strategic planning process for the clinical program(s)
- Demonstrates leadership skills, expertise, and in-depth knowledge base consistent with advanced nursing practice
- Implements change using relevant theory and data by applying quality improvement principles
- Utilizes principles of the consultative process to effectively communicate and contribute to the development of systems related to patient care, organization and health care outcomes
- Mandates internal / external referrals to facilitate best clinical practice
- Initiates and collaborates in the strategic planning for program development, implementation and evaluation across the organization for specific patient population
- Articulates and demonstrates the integration of the role through research, education, consulting and managing specific patient population
- Advocates for patients and peers by sensitizing interdisciplinary members to issues / dilemmas that require attention and consideration toward positive results
- Identifies research questions and conducts research to determine best practice
- Provides and critically analyses data relevant to system / program decision-making
- Participates in local, regional, provincial, national and international forums to develop policy and standards
- Influences nursing practice by stimulating critical thinking
- Acts as a research resource to the interdisciplinary team
- Analyzes and interprets current research / literature / knowledge applicable to health care practice with the identified patient population
- Identifies learning needs of peers and provides opportunities for continued education as it relates to clinical practice and patient care
- Disseminates findings in formats relevant to patients, peers and other colleagues
- Facilitates own professional development and maintains clinical competency through formal educational programs and monitoring opportunities with colleagues.
- Contributes to a transparent culture of client and staff safety by adhering to and abiding by patient and staff safety policies and procedures set by SJHC
- Promotes an environment that encourages and supports change using change leadership theory

Qualifications & Skills
- Successful completion of a Master’s degree from an accredited Nurse Practitioner Program
- Member in good standing of the College of Nurses of Ontario as a Registered Nurse in the Extended Class
- Professional Liability Insurance
- BCLS
- Wound care training and proficiency (preferred)
- Three years nursing experience in specialty area
- Membership in RNAO or NPAO
- Membership in professional organizations appropriate for specialty
- Demonstrated leadership and team work skills
- Excellent communication skills (verbal and written) and team building skills
