Relief Pharmacist (GTA)
Job Overview We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for our patients.
This position requires a strong background in hospital pharmacy, particularly in pediatrics and acute care settings. The Pharmacist will collaborate with healthcare professionals to provide exceptional patient care, manage medication administration, and educate patients about their medications.
Responsibilities Review and interpret physician orders for accuracy and appropriateness of medication therapy. Dispense medications accurately while adhering to safety protocols and regulations.
Provide clinical consultation to healthcare providers regarding medication therapy management. Monitor patient outcomes and adjust medication regimens as necessary to optimize therapeutic results.
Educate patients and their families on medication usage, potential side effects, and interactions. Maintain accurate records of medication administration and patient care activities.
Stay updated on medical terminology, drug information, and best practices in pharmacy care. Participate in interdisciplinary rounds and contribute to patient care discussions within the healthcare team.
Qualifications Doctor of Pharmacy (Pharm.D.) degree from an accredited institution. Valid pharmacist license in the state of practice.
Experience in a hospital pharmacy setting preferred, with a focus on pediatrics or acute care. Strong understanding of medical terminology and pharmacotherapy principles.
Excellent communication skills with the ability to work collaboratively within a healthcare team. Commitment to providing high-quality patient care and ensuring medication safety.
