The Quality Assurance and Specialized Services Team is looking for a qualified Transitional Age Youth Coordinator (TAY Coordinator). The TAY Coordinator provides expertise to individuals, families and caregivers looking to transition from children / youth services to Adult services. The TAY Coordinator engages multiple stakeholders within the identified circle of care team to ensure a smooth transition from children’s services to Ministry of Children, Community and Social Services (MCCSS). Supports include regular communication and check in meetings with service users and service providers to ensure identified goals are met and adjusted to changing support needs, administrative needs, identified budget and individual’s goals. The TAY Coordinator regularly coordinates with agency partners including Outside Paid Resources (OPR) and Transfer Payment Agencies (TPA) to explore potential residential / non-residential adult supports and services in line with the supported person’s goals. The TAY Coordinator is responsible for developing the initial individualized budget for supported person’s transition to adult services as well as ongoing correspondence with OPRs, TPAs and circle of care team regarding transition status, funding approval and additional ministry directions or updates. The TAY Coordinator engages in ongoing discussion and collaboration with the Quality Assurance Team, identified circle of care as well as external stakeholders to ensure individualized budget meets service user needs as identified in individualized plan.
