We are currently seeking a Vitality LifeCare Assessor to lead our assessment and care planning process, ensuring residents receive the best possible support through proactive, coordinated care.
About the Role
As a Vitality LifeCare Assessor , you will play a key role in conducting resident assessments, developing personalized care plans, and facilitating seamless coordination with our interdisciplinary team and allied health partners. Your expertise in care navigation and compliance will help optimize resident care and enhance their quality of life.
Key Responsibilities
- Conduct comprehensive resident assessments using S|R|R’s Comprehensive Geriatric Assessment (CGA) tool within Point Click Care (PCC) .
- Develop and update person-centered care plans in collaboration with nurses, allied health partners, primary care providers, and families.
- Lead Resident & Family Care Conferences and interdisciplinary meetings to support holistic, team-based care.
- Monitor resident health status and coordinate preventative, therapeutic, and rehabilitative services .
- Ensure all assessments and care plans comply with regulations and best practices.
- Serve as a key care navigator , assisting with referrals, transitions, and follow-ups with healthcare providers, hospitals, and community services.
- Advocate for residents by addressing their care preferences and engaging families in decision-making.
- Collaborate with the Life Enrichment and Dietary teams to ensure a holistic approach to well-being.
- Support compliance by ensuring all care plans are reviewed, signed, and submitted in a timely manner.
What We’re Looking For
Registered Nurse (RN), Registered Practical Nurse (RPN), or equivalent healthcare background with experience in senior care.Strong knowledge of geriatric assessments, care planning, and care coordination .Experience with Point Click Care (PCC) or other electronic health records is an asset.Excellent communication and leadership skills to collaborate effectively with residents, families, and interdisciplinary teams.Knowledge of compliance standards, CNO best practices, and privacy regulations .Ability to manage multiple priorities in a fast-paced environment with compassion and professionalism.Why Join S|R|R?
