Reporting to the Manager, Pharmacy Oncology, the Pharmacist, Oncology is an integral member of the Pharmacy and Cancer Care Team. The Oncology Pharmacist is accountable and responsible for completing oncology regimen assessments, ensuring optimal cancer medication therapy and minimizing the potential for adverse events. They are also accountable for providing accurate medication information to the interdisciplinary team including reconciling patient medication lists and providing patient education. The pharmacist promotes safe and appropriate use of cancer medication therapy in a multidisciplinary setting to minimize risk and optimize health care resources as they pertain to medications.
Education :
- Registered Pharmacist, in good standing with the Ontario College of Pharmacists (Part A)
- Bachelor of Science degree in Pharmacy (PharmD, an asset)
- Advanced Oncology for Pharmacists (U of T), an asset – required within five (5) years of hire
Experience :
Post-graduate Pharmacy Residency Program (ACPR), an assetMinimum of two (2) years of experience in a hospital pharmacy setting, preferredMinimum of two (2) years of experience in pharmacy oncology, an assetCompetencies :

Complete patient oncology medication assessmentsEnsure optimal cancer medication therapy and minimization of potential adverse events.Review of lab and test information to ensure appropriate cancer therapy.Participate in patient care within the multidisciplinary team.Participate in cancer drug distribution, including accurate order entry of oncology agents.Participate in medication list reconciliation and provide relevant drug information to the multidisciplinary teamParticipate in the evaluation of complimentary treatments within the oncology setting.Participate in patient education.Evaluate CCO NDFP and QBP funding for oncology regimens to be administered to patientsActively review and provides recommendations on various document submissions relating to cancer careParticipate in the education and mentoring of pharmacy students and new staff.Participate in the education and ongoing learning of nursing staff.Participate in duties related to Oncology Clinical Trials : Initial training and amendments, review and dispensing of investigational productsOther duties, as assigned.Unless otherwise stated applicants must have a minimum educational requirement of Grade 12 or equivalent and must demonstrate proven workplace excellence through commitment to strong job performance and attendance.
