Provide Case management to patients living with a chronic and persistent mental illness. The successful candidate has current registration with their respective college and demonstrates strong leadership skills. As part of an interdisciplinary team, you will provide assessments, treatment planning for clients, medication monitoring and administration.

Hours: Monday to Friday - 7.5 hour shifts - 08:30 - 16:30, except Tuesday which an evening shift 1100-1900 (hours subject to change based on operational needs)
Salary: $47.039 to $60.979 per hour
