Overview:

The Homelessness, Addiction, and Recovery Treatment (HART) Hub is a collaborative initiative designed to provide a seamless continuum of care through a culturally responsive and healing-centered model. Tailored to meet the needs of individuals facing homelessness, and mental health challenges, the Kenora HART Hub serves as a vital anchor for support within the Kenora geographical area, and surrounding communities. By delivering coordinated, compassionate, and evidence-based services, the HART Hub ensures equitable access to care. This approach integrates holistic and culturally grounded practices, to promote stability, healing, and long-term recovery for members who are most vulnerable within our communities.

Reporting to the HART Hub Operations Coordinator and Chief Executive Officer of CMHA Kenora Branch, the Crisis Stabilization Worker will support individuals 18 years and older by accessing services by providing a psychosocial response to promote crisis resolution and empowerment of the individual accessing the HART Hub. The Crisis Stabilization Worker will collaboratively work with the co-located integrated care team and external partners in assessing and identifying those requiring crisis support by assessing their needs and linking them to the most appropriate support and services based on their needs. The Crisis Stabilization Worker will support individuals accessing the HART Hub by receiving referrals through the co-located nursing support at the HART Hub that are medically stable by providing short short-term stabilization supports that would assist in transitioning individuals into longer-term stabilization supports (e.g. Safe Bed Program, Hospital). Hours of work will be Monday to Friday from 12:30 p.m.-8:00 p.m.



Qualifications:

BSW, MSW or registration with the College of Nurses of Ontario

Registration or eligible for registration with a regulatory College

Experience working with individuals who are homeless and marginalized who also have mental health and addiction struggles and/or demonstrated ability to work with this population

Experience in mental health crisis intervention

Training in crisis intervention and crisis resolution

Extensive knowledge of community resources, service and support programs

Working knowledge of a variety of treatment, intervention and support strategies for mental health and addictions

Excellent verbal, non-verbal and written communication skills, strong skills and ability to provide written communication

Crisis intervention and prevention skills

Identifies problems and uses related problem-solving skills

Ability to assess level of risk

Personal qualities of empathy, understanding, and patience

Good interpersonal communication and relationship building skills

Ability to communicate professionally with community agencies

Cultural competence for diverse populations

Understanding and applied knowledge of Recovery Model

Understanding of trauma informed practice and implementation

Good documentation skills

Proficient computer skills

Demonstrated understanding of personal space and ability to role model healthy boundaries

Ability to manage conflict and crisis situations

Demonstrated time management skills

Ability to work independently as well as in a team environment

Creative problem solving

First aid & CPR/AED, Mental health first aid, and NVCI or willing to obtain

Valid Drivers License

Duties:

  • Adherence to all policies and procedures of Canadian Mental Health Association, Kenora Branch
  • Complete mental health and addiction assessments with partner nursing staff to identify the impact of psychiatric symptoms of functioning, risk factors related to safety of client and others, and opportunities for de-escalation and stabilization
  • Establish a therapeutic relationship to maximize the benefits of engagement with a client accessing the crisis stabilization bed
  • Provide short-term follow-up and support to bridge the gap from referral to engagement with a longer-term support service (Safe Bed Program, Hospital)
  • Advocate as necessary to facility access to services and support with the objective of remediating immediate crisis situation
  • Engage emergency services when it is determined that additional intervention is required
  • Assess indicators for crisis involvement (inability to access treatment, transportation barriers, finances, housing etc.)
  • Obtain information as needed from relevant sources where appropriate and applicable
  • Interact and provide support and assessment in a non-judgmental fashion and provide care that demonstrates sensitivity to individuals diversity (e.g. culture, race, age, sexual orientation, gender, beliefs and values)
  • Support individuals in developing skills for self-advocacy to learn new behaviors and develop coping skills by fostering healthy interdependence through limit setting
  • Provide health teaching and information regarding coping techniques and strategies (e.g. grounding, breathing techniques, self-care, nutrition, sleep hygiene, techniques to enhance overall well-being
  • Provide short-term, psychotherapeutic intervention to assist individuals in gaining insight and improving coping skills within the current crisis
  • Refer individuals for additional assessments as is determined to be necessary and/or appropriate
  • Assist the individual to identify and connect with personal/natural supports where feasible and appropriate
  • Provide follow-up support and/or check ins to bridge the gap from referral to engagement with a longer-term support service as appropriate
  • Demonstrate knowledge of community resources
  • Assist in building positive working relationships across the service system (community, hospital, detox, Safe Bed Programs)
  • Participate in public education and advocacy
  • Facilitate a coordinated appropriate to service provision
  • Screening of HART Hub clients using approved clinical assessment tools to determine needs and triage to appropriate services (VI-SPDAT, OCAN Assessment etc.)
  • Ensure clients who are accessing the HART Hub are triaged to the Clinical Service Hub Team at the Kenora Emergency Shelter where appropriate (Clinical Service Hub Team Lead, Service Navigator and Peer Support)
  • Ensure clients are triaged to appropriate services that are co-located onsite by the integrated care team services which will consist of low barrier access to a range of services through case management and transition support, nursing, peer support, transportation support, crisis stabilization support, addiction services, land-based cultural care, connection and referral pathways to supportive housing support
  • Develop an integrated Recovery Plan in partnership with the identified integrated care team and strategies to support the achievement of goals and objectives of clients of the HART Hub
  • Assertive engagement with clients of the HART Hub to connect them to services
  • Make appropriate recommendations for support and services
  • Consult and collaborate with the integrated care team regarding the support and integrated recovery planning for HART Hub clients
  • Liaise with the integrated care team to assess client needs and the community
  • Encourage understanding and support for clients by developing and maintaining working linkages with law enforcement, primary care, human service agencies, and with informal community resources
  • Continuous assessment of risk
  • Continuous assessment of risk related to the clients housing needs, substance use, mental health needs, social service needs and employment needs
  • Advocacy or linking of clients with appropriate services, support and resources to meet the goals and objectives of the integrated recovery plan
  • Conducting regular formal or informal evaluation of client satisfaction, achievement of goals, reassessment of service requirements and or needs in consultation with the client
  • Direct provision of side-by-side support with clients where appropriate
  • Provision of supportive counselling where appropriate
  • Assist clients and provide modeling for problem solving and priority setting
  • Maintenance of client files consistent with policies and procedures of Canadian Mental Health Association, Kenora Branch
  • Outcome monitoring and evaluation as directed by the Chief Executive Officer
  • Professionally representing Canadian Mental Health Association, Kenora Branch
  • Participation in administrative and office functions, including ongoing maintenance of office space
  • Participation in educational and professional development opportunities
  • Ensure safety and security of all HART Hub clients during shift, including interpersonal safety and security on the HART Hub services and integrated care team while onsite
  • General administration duties
  • Communicate changes and new rules/regulations to staff; ensure all operation and program policies and procedures are being followed
  • Attend education/training opportunities as required
  • Continue to build relationships within the community through one-on-one networking, group meetings, and public outreach as directed by Chief Executive Officer
  • Participate on specific committees or working groups as assigned by Chief Executive Officer
  • Provide a safe environment by ensuring adherences to Workplace Health and Safety
  • Actively participate in the development of sustainability of the ongoing Accreditation Standards, Required Organizational Practices and Quality Improvement projects
  • Regulations and Policies, Infection Control Guidelines, WHIMIS and Safe Work Procedures, including but not limited to Fire, Disaster and Evacuation Plans
  • Ensure client safety and client safety activities are emphasized throughout orientation of new staff members
