Job Title: Registered Nurse

Employment Type: Casual

Employee Group: ONA

Shifts: 12-hours, including days and nights (7AM-7PM / 7PM-7AM)

Payrate: $35.44 - $44.18 per hour

Stedman Community Hospice at St. Joseph's Lifecare Centre Brantford is seeking a Registered Nurse to join our team. Our 10-bed residence gives people who are living with a palliative diagnosis the opportunity to live with dignity and comfort in a place that feels like home.


The Registered Nurse will assume a lead role in providing high quality end of life care to the residents at Stedman Community Hospice. As part of the interdisciplinary team, the RN contributes to the development, implementation and communication of the resident’s care plan and reports any substantial findings and/or changes in resident’s condition and actively contributes to solutions. It is expected that the RN will be a positive example and role model and will promote a resident focused, goal-oriented approach to care, nurturing a home like environment and in keeping with the Mission, Vision, and Values of St. Joseph’s Lifecare Centre/Stedman Community Hospice.

Duties:

Perform all duties in accordance with the “Standards of Practice of the College of Nurses of Ontario” in providing direct and indirect nursing care as required in collaboration with other health care workers. Duties may include but are not limited to: Effective leadership using the nursing process to access, plan, implement and evaluate nursing care; advocating and supporting the patient and/or their SDM in achieving their directive for care, and any other duties as assigned.

Key Accountabilities and Success Criteria

  • Gives direction for all aspects of residents care and giving oversight to other members of the care team in accordance with the Regulated Health Professionals Act and the College of Nurses of Ontario Practice Standards
  • Recognizes, assesses and responds to resident health status changes
  • Provides leadership to the team, working collaboratively with them to assess workload, assign caseload and priorities each shift
  • Performs physical, psychosocial, spiritual and emotional assessment of the resident, including their families
  • Creates an environment and provides opportunities for the person and their family to exercise religious, cultural and spiritual rituals, customs, rites and beliefs throughout the person’s illness and through bereavement
  • Responds to ethical issues using ethical framework (YODA) and anticipates and addresses ethical and legal issues that may be encountered when caring for individuals with life-limiting conditions
  • Ensures all medications are processed and recorded according to legislative and regulative requirements
  • Administers medication and treatments as required/ordered, monitors for effectiveness and side effects

Qualifications and Education Required for This Position:

  • Graduate of an approved Registered Nursing program.
  • Registered with the College of Nurses of Ontario and currently in good standing.
  • Demonstrates strong positive personal and interpersonal communication skills, both verbally and written. Ability to work as a member of a multi-disciplinary team.
  • Current CPR and First Aid certification will be required.
  • Ideal candidates will have “Fundamentals of Palliative Care”. In addition, Enhanced Fundamentals and the Comprehensive Palliative Education Program (CAPCE) and will have or be pursuing their CNA certification in Hospice Palliative Care Nursing.
  • Minimum of 2-years of experience in nursing, knowledge of hospice and palliative care is essential.
  • Strong clinical practices with ability to champion evidence-based best practices particularly in pain and symptom management and end-of-life care.
  • Demonstrated excellent problem-solve skills with ability to manage complex clinical situations and psycho-social circumstances in providing or managing the needs of palliative patient’s across the continuum of end-of-life care.
  • Ability to recognize, assess and treat end-of-life care emergencies and psychosocial distress.
  • Ability to recognize safety issues, follow safe work practices, advocate for client care and safety concerns.
  • Demonstrates a patient/family-centered approach to nursing care. Recognizes the importance of establishing authentic and empathetic professional relationships with patients and families.
  • Ability to understand and document patient and family preferences, goals of care, and act as an advocate to help patients meet their care needs.
  • Ability to collaborate with other members of the health care team to access required services and to ensure smooth transitions.
  • Demonstrated ability to use knowledge-transfer techniques to support practice delegation in addition to completing ongoing health teaching with both patients and families.
  • Demonstrated commitment to modelling lifelong learning and professional growth.

Physical Demands:

While performing the duties of this job, the employee is regularly required to stand, walk and assist with patient lifts/repositioning. The employee is frequently required to reach with hands and arms, stoop, kneel and/ or crouch. The employee is occasionally required to sit, climb or balance. The employee must follow transfer and lift policies for safe practice, but also be able to lift and/ or move up to 75 Ibs.
