Registered Nurse, Primary Care - Tri-Cities

Salary CAD $41.42 - $59.52 per hour

Responsibilities

  • Provides direct nursing care to clients receiving scheduled or urgent/timely primary care; assesses clients, collects information, and develops a nursing diagnosis to determine appropriate interventions in collaboration with other clinical staff.
  • Processes physician or nurse practitioner orders, initiates standing orders, administers medications, carries out treatments, and monitors/document effects.
  • Establishes a therapeutic relationship with clients to support autonomy, dignity, and privacy in decision-making and care planning.
  • Screens referrals, conducts individualized client assessments, and coordinates inter-professional care planning and interventions when appropriate; facilitates timely health services and transitions across the care continuum.
  • Develops a comprehensive shared client care plan with the inter-professional team and supports transitions to primary/community care providers or community agencies.
  • Delivers direct planned and unplanned care, provides explanations of care to clients and families, and ensures access to appropriate care.
  • Reviews and updates care plans to improve client outcomes; initiates next steps in collaboration with clients and families and supports transition across the care continuum.
  • Collaborates with clients and families to discuss clinical findings, set goals, and plan action steps; uses system information to inform the care plan and priorities.
  • Supports clinic visits by providing information and advocating for client choices; assists transitions to other programs or providers as needed.
  • Performs outreach nursing care in home or alternate settings, including assessment and evaluation of responses to medications and plans of care for clients and families.
  • Provides health counselling and education to clients for self-management, wellness, and prevention across clinic, telephone, or home visits.
  • Maintains clinical records and statistical information according to policies and procedures.
  • Engages in ongoing professional development and participates in quality improvement, risk management, and education activities as required.
  • Supports orientation and ongoing education of nursing staff and students and contributes to the development and revision of standard practices and quality initiatives.
  • Performs other related duties as assigned.

Qualifications

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • One (1) year recent related clinical experience in acute care, urgent/emergency care, or a related clinical area including assessing and treating children, youth, and adults in acute or community/outpatient settings, or an equivalent combination of education, training and experience.
  • Valid BC Driver’s license and access to a personal vehicle for business-related purposes, as required.
