Join\u0026nbsp;Pro Vita Care Management\u0026nbsp;and become part of a supportive team committed to providing high-quality care.\nSocial Worker Must Haves:\u0026nbsp;\n\nGraduation from a Bachelors of Social Work Program.\nHolds a registration with the BC Association of Social Workers\nSatisfactory proof of COVID-19 vaccinations.\u0026nbsp;\nPasses and complies with Ministry of Justice Criminal Record Check upon hire and every 5 years thereafter.\nAbility to effectively communicate in English both verbal and written.\u0026nbsp;\n\nSocial Worker Nice to Haves:\u0026nbsp;\n\n2 years experience in a social worker capacity.\u0026nbsp;\nGerontological/ Senior's care environment experience.\u0026nbsp;\nMaintains First Aide and CPR {CONTENT}quot;A{CONTENT}quot; certification.\nProven experience working unsupervised and providing direct supervision.\u0026nbsp;\n\nSocial Worker\u0026nbsp;Responsibilities:\n\nInterviews prospective residents and their families to determine care needs and provide appropriate counselling to ensure a smooth transition.\nLiaises with outside agencies and health authority to facilitate the planning and scheduling of resident admissions, discharges and transfers.\nPlans, schedules and facilitates resident admissions, transfers and discharges to maximize occupancy and resident/family satisfaction.\nProvides counselling and support to residents and families experiencing transitional distress.\nPlans, develops, organizes, and facilitates resident/family supports, through contact with community health agencies, support organizations and establishment of Resident/Family Council.\nParticipates in care conferences and multidisciplinary team meetings.\nMaintains appropriate clinical records, statistics and resident health documentation.\nIn collaboration with the care manager, provides guidance to staff by promoting new social work initiatives; developing, interpreting and implementing related policies \u0026amp; procedures, and providing in-service education.\nDevelops, implements, monitors, and evaluates social work service programs, policies \u0026amp; procedures and continuous quality improvement initiatives.\nAs a member of the site leadership team, participates in discussions and decision-making related to health \u0026amp; safety, resident care, financial and human resource management and general operations.\u0026nbsp;\n\nIf you are a kind, caring individual who enjoys connecting with and making a difference in the lives of elders \u0026ndash; Apply Today!\n*Please note that Healthcare wages are being adjusted and topped up by the government currently, August 2024\n\n\n\n\n\n"}" data-react-cache-id="AIStudio/AIInterviewModal/AIInterviewModal-0">Personal Details * Required field
