Program Summary: M-DOT is a specialized team of providers from LOFT, Inner City Health Associates (ICHA) and the Centre for Addictions and Mental Health (CAMH), that delivers services to the most vulnerable individuals living and sleeping on the streets of Toronto.

The Team is comprised of 1 Team Lead (LOFT), 5 case managers (LOFT), 1 peer support worker (LOFT), 2 psychiatrists (ICHA), 1 nurse (ICHA) and 1 Concurrent Disorders Social Worker (CAMH).

M-DOT provides clinical hands-on support to those with complex healthcare needs, who are experiencing chronic homelessness.

M-DOT provides outreach and case management and peer support services with access to psychiatric and medical assessment, consultation, access to treatment, service planning and advocacy.

Services are designed to be short-term, focused on comprehensive assessment, stabilization and linking to long term resources.

M-DOT provides services to individuals often identified as ‘complex’, who have not responded positively to other programs or attempts to engage.

The aim of M-DOT is to provide a low barrier transitional service, that meets individuals where they are at and assists in stabilizing their health, mental health and substance use. M-DOT works from a housing first and harm reduction approach.

The goal is to meet immediate needs, reduce barriers and assist individuals in getting connected to housing and follow up support that meets the level of need required for each individual to live successfully in the community.

  • Job Summary: The Case Manager on the M-DOT Hotel Team provides direct service to homeless individuals living with mental health, health, and addictions issues.

The position provides both on site and community-based assessment and case management services to individuals living in various physical distancing shelter hotel programs through the city of Toronto.

Case Managers work as part of a team that includes Team Leaders, Peer Support Workers, and Housing Support Workers, to meet clients’ housing and other social and health needs.

The program works closely with nurses and physicians from Inner City Health Associates, to assess, plan, treat and support clients.

The position will involve driving and a valid Ontario driver’s license and insurance are required.

Access to a car is a requirement.

What You Will Do: Create individualized support and recovery plans with clients and professionals based on assessments.

Support clients in building independence by teaching daily living skills (e.g., hygiene, medication, cooking, budgeting).

Coordinate care by connecting clients with social, medical, and community support.

Provide crisis intervention and help develop crisis plans.

Represent the program professionally with clients, families, and community partners.

Mediate conflicts and help clients learn problem-solving skills.

Organize or support social activities to reduce isolation.

Maintain accurate client records and documentation.

Work collaboratively with the team, contribute ideas, and support new staff or volunteers.

Provide on-call 24/7 crisis support on a rotating basis.

Assist with preparing units for new clients.

Provide case management and connect clients with services like housing and ID support.

Help clients access financial supports such as Ontario Works (OW) and Ontario Disability Support Program (ODSP) or other financial resources as required.

Other duties as assigned by Program Manager and/or designated supervisor.

What You Bring: Post-secondary education in Social Services / Human Services or equivalent field.

Minimum 3 years’ working experience with client population group.

Understanding of challenges faced by adults with challenges, eg: mental illness, addictions, physical health concerns, and homelessness etc.

Proficient in intake assessment, crisis intervention, advocacy, and problem-solving to support client needs.

Ability to build therapeutic relationships and collaborate with clients on goals.

Familiarity with community resources and professional services.

Strong teamwork skills in a multidisciplinary environment.

Strong interpersonal, communication, and counselling skills to effectively engage clients in sensitive areas such as life skills, healthy living, and relationships.

Solid computer literacy for maintaining records, files, and reports.

Effective analytical, organizational, and time management skills for coordinating client support and program activities.

Demonstrates cultural competency, with an understanding of diverse backgrounds and inclusive practices.

Must have a valid driver’s license, insurance, and a reliable vehicle to travel within the region.

Work Environment: Expected to meet people, on site and outside, making exposure to the weather and climbing stairs are expectations of the position.

A car is an asset.

Need to walk on rough terrain from time-to-time, for example, uneven ground, parks, or ravines while providing outreach to community members sleeping outside.
