In this role, you'll work closely with them to uncover their unique needs, strengths, and aspirations - helping to turn goals into meaningful opportunities. Your support empowers individuals to build on their assets and live a life that is both fulfilling and self-directed.Responsibilities include:Supporting and facilitating access to generic services, informal supports, and CLBC-funded services and allocation of small targeted amounts of fundingAssist the individual and their family in identifying the activities and resources required to reach their goals, by supporting the individual to explore their capacity and enhance their ability to make informed decisionsProviding information about informal community supports, generic services, CLBC services, and peer and family support services availableCollaborating with other service providers to assist the individual in responding to crises and resolving problemsCoordinating servicesCreating and implementing individual support plans, which include informal and formal safeguardsAssist individuals and families in connecting to their community to build on the strengths and assets of the individualConfirming eligibility for CLBC-funded servicesWhat you will bring:Preference will be given to candidates with a Bachelor of Social Work or an equivalent degreeExperience working in the Community Living field with children, youth or adults with developmental disabilities and their familiesAn equivalent combination of relevant education/experience may be consideredWe're also looking for:Strong verbal and written ability to connect effectively with individuals and familiesCollaborative planning ability to partner with individuals and their support circle to create meaningful, person-centred plansAbility to apply critical thinking and sound judgment to develop and implement effective solutionsPerson-centred approach to ensure the individual's voice is central in all decisionsAbility to foster positive, productive relationships with individuals, families, and community partnersAbility to manage multiple priorities and deliver results within deadlinesCreative & flexible thinking while working within a policy framework to identify support options tailored to individual needsAdditional Info:Possession of a class 5 driver's license and a clean driver's abstract are required.Successful applicants are subject to a Criminal Record CheckAn eligibility list for internal candidates may be establishedAuxiliary employees receive vacation pay of 6% of their regular earnings, paid bi-weekly, plus $0.91 per working hour, up to $63.70 per bi-weekly pay period, in lieu of sick time and benefits. After 90 days of employment, you are entitled to up to five days of paid illness and injury leave per year. You will begin contributing to the Public Service Pension Plan once your pensionable earnings exceed half of the year's maximum, typically after about six months of full-time employment.Closing date: September 16, 2026If your application is shortlisted, you'll be invited to an interview where you'll have the opportunity to share more about your experience.
