Job Summary

Reporting to the Client Care Coordinator, the Home Support Worker is part of a healthcare team that provides assistance to families or persons during times of illness, disability, or inability to properly care for themselves. The worker supports client independence and helps them remain in their own homes through assistance with activities of daily living.

Summary Of Essential Job Functions

• Provide personal care such as bathing, toileting, and grooming to the client
• Complete housekeeping tasks, meal preparation, and shopping
• Provide companionship and accompany the client to community events, activities, and appointments
• Follow written plan of care as approved by the Client Care Manager
• Provide specialized care such as Alzheimer or palliative care, if required
• Document and report client progress
• Promote good mental health for the client
• Develop a caring relationship with the client and their family
• Provide emotional and psychological support
• Ensure a healthy and safe living and working environment while following agency guidelines on lifting and transfers
• Adhere to and enforce CareGivers’ Code of Conduct
• Report all occupational health and safety hazards and follow OH&S guidelines
