Position Summary

Registered Nurse, Intake, Adult Mental Health Outpatient Program

Your goal in this position is to perform, intake for an adult mental health population, collecting collateral information from patients, physicians and community agencies to determine suitability for services in the Adult Mental Health Outpatient program. You will perform phone / walk-in triage / intake interviews and refer to appropriate resources either at North York General Hospital or other agencies, based on acuity of symptoms and history of illness. You will maintain, update resource’s information and contacts and monitor the waitlist for services. You will also be responsible to determine urgency of referrals using established criteria, re-direct callers to suitable resources, consult members of the interdisciplinary team regarding issues related to the intake and provide short term counselling to a specified population.

You will be required on as needed basis to assist with providing coverage in the Wellness & Long Acting Injection Clinic for referred patients.

You will be responding for conducting the Point of Care Testing (POC) for clients who are on Clozaril and document same.

You will also participate in program and hospital committees and meetings, as required, collate and share up-to-date resource information and liaise with other agencies.

You will deliver patient / family driven care with an emphasis on assessment, education, prevention, maintenance, and restoration of health in a supportive and culturally sensitive manner. You’re a great relationship builder, and foster close contacts with peers across the entire organization. You have great interpersonal skills, and effortlessly connect with physicians, staff, patients, and families. You will take on the shared responsibility for ensuring that health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers and families.

  • Demonstrate the ability to function to the full scope of nursing practice per the College of Nurse of Ontario
  • Communicate effectively (written and verbal) with patients / clients, families and other members of the health care team

  • Demonstrate knowledge of transfers and lifts
  • Be committed to ongoing learning, professional development, and continuous quality improvement
  • Assume a leadership / charge role with demonstrated evidence of self- motivation regarding continuing education in areas of patient assessment, patient education and related Clinical Specialties
  • Foster good public relations and working relationships in a family centered care environment
  • Demonstrate your ability to perform appropriately in emergency situations
  • Qualifications

  • Diploma in Nursing, Baccalaureate Degree in Nursing
  • Registered and in good standing with the College of Nurses of Ontario
  • Current Basic Cardiac Life Support (BCLS) certification
  • 3-5 years’ experience in adult mental health and working with families
  • Certificate in Mental Health Nursing from a recognized community college, preferred
  • Certification in Crisis Intervention by Crisis Prevention Institute (CPI), preferred
  • Canadian Nurses Association (CNA) certification in Psychiatry - Mental Health Nursing, preferred.
  • Working knowledge of the network of the outpatient services and community-based resources for adults with mental health needs, preferred
  • Previous intake experience, preferred
