Hours of Work : Days (6 : 30AM-2 : 30PM), Evenings (2 : 30PM-10 : 30PM), Nights (10 : 30PM-6 : 30AM)
Job Summary :
Reporting to the Director Long Term Care (or delegate), the Personal Support Worker (PSW) utilizes the Enhanced Living Model to assist residents with personal care and activities of daily living in the assigned neighborhoods by following the individualized care plans.
Key Accountabilities & Success Criteria :
Performs all duties in accordance with the Mission Statement and policies and procedures of SJLCB as adopted from the Ministry of Health guidelines.

The Provision of Resident Care
assisting with personal hygiene (bathing, dressing, hair care, mouth care, nail care, and shaving)assist with nutrition (providing fluids and nourishments as required, meal set up, assistance with feeding, and reporting of intake. Weights to be taken monthly and changes to be reported immediately.)assisting with urinary and bowel elimination (toileting, disposable briefs, emptying of urinary and colostomy drainage bags, and reporting of output)promoting comfort by assisting with positioning, mobility, and ambulation as per care plan (positioning and turning of residents; using mechanical lifts appropriately; and reporting of any changes)promoting skin integrity (providing good skin care through washing, positioning, and turning of residents; ensuring that environment is safe of obstacles to prevent injuries; applying dressings to stage 1 skin ulcers and reporting any changes in skin integrity). Immediately correcting if possible and reporting any situations that can or will affect the health and safety of the residentsassisting with exercisesfacilitating appropriate rest and socializationEstablishing and Maintaining Healthy Interpersonal Relationships

Must possess a Personal Support Worker certificate or diploma from Ministry of Health of Ontario, Long Term Care Division approved educational facility.BCLS certification required.Strong organizational and prioritization skills as well as demonstrated teamwork skills.Must possess a clear understanding of ethicsDemonstrated written and verbal communication skills as well as excellent interpersonal skillsExcellent problem solving and conflict management skills are essential.AVAILABILITY :
