Job description
A not-for-profit organization is seeking a Transitional Care Assistant to support client independence by facilitating services from hospital to home. The role involves working with caregivers and health teams to ensure clients' needs are met safely while respecting their choices. Candidates must have a post-secondary certification in gerontology or social services, a valid driver's licence, and experience. Full vaccination against COVID-19 is mandatory. This position involves in-person work in Elliot Lake, ON.
